白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障的疗效观察

摘　　要：目的观察超声乳化白内障吸除联合折叠式人工晶体植入术治疗闭角型青光眼合并白内障患者的临床疗效。方法收集2010年6月～2011年6月于我院眼科中心收治的闭角型青光眼合并白内障71例（71眼），患者均接受超声乳化白内障吸除联合折叠式人工晶体植入术，术后随访6个月。结果术后最佳矫正视力较术前显著提高（P〈0．05）；患者术后3个月和6个月的眼压平均为（10．2±4．6）mmHg和（10．7±4．9）mmHg，较术前用药前后眼压相比均显著下降（P〈0．05）；超声生物显微镜检查发现术后前房深度较术前均显著增加（P〈0．05）；术后患者房角均较术前增宽，房角粘连关闭象限不同程度开放。结论超声乳化白内障吸除联合折叠式人工晶体植入术可有效降低眼压、提高视力，为闭角型青光眼同时合并白内障患者安全有效的治疗途径。Objective To investigate the clinical effect of phacoemulsification with foldable posterior chamber in Intraoeular lens implantation （PC-IOL） for the treatment of angle-closure glaucoma （ACG） with cataract. Methods A total of 71 eyes of 71 eases with ACG and cataract admitted to our hospital from June 2010 to June 2011 were retrospectively ananlysed. After phaeoem ulsification with foldable PC-IOL implantation, the patients were followed up for 6 months. Results The postoperative best corrected visual acuity of all patients had improved significantly （P 〈 0.05）; the average intraoeular pressure of （10.2± 4.6） mm Hg and （10.7 ±4.9） mm Hg for 3 months and 6 months after treatment were significantly lower than those before treatment （P 〈 0.05）; postoperative anterior chamber after treatment was significantly deeper than that before treatment ultrasound biomieroscopie examination（P 〈 0.05）; it was found that anterior chamber width increased significantly （P 〈 0.05）. Conclusion Phacoemulsification with foldable PC-IOL implantation substantially reduced intraocular pressure and improved visual acuity and can be a good alternative in treatment of ACG with cataract.
