保留回盲肠瓣一期手术治疗左半结肠癌急性梗阻的疗效

摘　　要：目的研讨保留回盲肠瓣一期手术治疗左半结肠癌急性梗阻的可行性及临床疗效。方法选取本院2010年8月～2011年12月收治的左半结肠癌发生急性肠梗阻的患者46例,随机分为两组,甲组患者22例,采用保留回盲肠瓣一期手术治疗;乙组患者24例,采用不保留回盲瓣一期手术治疗。对比分析两组患者的术后疗效及并发症发生情况。结果两组患者术后肠梗阻情况均得到不同程度的缓解,甲组患者疗效显著,与乙组患者疗效对比,差异有统计学意义(P<0.05)。甲组患者术后并发症的发生率显著低于乙组患者,差异有统计学意义(P<0.05)。结论依据患者的情况,采用保留回盲肠瓣一期手术进行治疗左半结肠癌急性梗阻的疗效较好,有利于术后肠道血运恢复,有利于术后患者的康复。Objective To study the feasibility and clinical effect of retention of ileocecal flap operation for the treatment of left colon cancer with acute obstruction. Methods Forty-six cases of left colon cancer with acute obstruction in our hospital from August 2010 to December 2011 were randomly divided into two groups, 22 patients of group A were given preservation of the ileocecal valve in one stage operation for treatment, 24 patients of group B were not given preservation of the ileocecal valve in one stage operation for treatment. The curative effect and the complication of two groups of patients in postoperative were compared and analyzed. Results The condition of intestinal obstruction of two groups of patients were obtained with different degree of ease, compared with the curative effect of group B, there was statistical significance （P 0.05）. The incidence of postoperative complications of group A was significantly lower than that of group B, there was significant difference with statistical significance （P 0.05）. Conclusion According to the condition of patients, the use of retention of ileocecal flap in one stage operation for the treatment of left colon cancer with acute obstruction is better, it has the benefit of the intestinal blood flow restoration and rehabilitation of patients in postoperative.
