适形放疗联合化疗与单纯化疗对局限期小细胞肺癌的疗效对比

摘　　要：目的探讨适形放疗联合化疗治疗局限期小细胞肺癌的治疗效果。方法选取2009年1月至2011年12月我院局限期小细胞肺癌患者76例作为研究对象,将所有患者根据治疗方法的不同随机分为适形放疗联合化疗组和单纯化疗组,每组各38例。观察比较两组患者治疗后的总有效率和毒副反应发生情况。结果联合治疗组总缓解率为92.11%,显著高于单纯化疗组的78.95%,两组间的差异具有统计学意义(P<0.05),而无变化的发生率均显著低于对照组(P<0.05)。两组均无进展病例发生。联合治疗组急性放射性肺损伤、放射性食管炎发生率分别为34.21%(13/38)和73.68%(28/38),恶心、呕吐的发生率为86.84%(33/38),白细胞减少率为71.05%(27/38);而化疗组恶心呕吐率和白细胞减少率分别为84.21%(32/38)、65.79%(25/38),两组间的差异均无统计学意义(P>0.05)。结论适形放疗联合化疗治疗局限期小细胞肺癌能够大大提高患者的治疗效果,值得在临床上广泛应用。Objective To explore the chnical effects of confomaal radiotherapy combined with chemotherapy for linfitedstage small cell lung cancer. Methods 76 cases with small cell lung cancer treated in our hospital from January 2009 to December 2011 were selected in the study. All patients were randomly divided into conformal radiotherapy plus chemotherapy group and single chemotherapy group according to different treatment strategy. Each group contained 38 cases. The overall efficient and the adverse reaction of two groups were observed and compared after treatment. Results The total complete response rate of conformal radiotherapy combined with chemotherapy group was 92.11%, significantly higher than that of control group （78.95 %）, with statistical difference （P 〈0.05）. The case rate of no change was significantly lower than that of control group （P〈0.05）. There were no progress cases in two groups. In the combination group, the incidence rate of acute radioactive lung injury and radioactive esophagitis were respectively 34.21% （13/38） and 73.68% （28/38）; the incidence rate of nausea and vomiting was 86.84% （33/38）, and the white blood cells reducing rate was 71.05% （27/38）, while they were 84.21% （32/38） and 65.79% （25/38） respectively in chemotherapy group. The difference between the two groups was not statistically significant （P 〉 0.05）. Conclusion Conformal radiotherapy combined with chemotherapy treatment used for limited-stage small cell lung cancer can greatly improve the curative effect. It deserves widespread use in clinics.
