摘 要:目的研究COX-2和Survivin与鼻咽癌放疗敏感性的相关性。方法收集2008年1月~2010年1月行鼻咽部活体组织病理检查确诊为鼻咽癌患者标本,免疫组化技术检测COX-2和Survivin在鼻咽癌放疗敏感者和不敏感者中的表达情况,对其作为预测放疗敏感性的分子指标进行有效性评价,鼻咽炎组织作为对照组。结果 COX-2在放疗敏感者及放疗不敏感者表达率分别为75.9%和50.0%,差异有显著性(P=0.000);Survivin的表达率分别77.6%和57.1%,差异有显著性(P=0.000)。COX-2预测放疗敏感性的灵敏度为75.9%,特异度为57.1%,Youden指数为0.33,准确度为72.2%,阳性预测值和阴性预测值分别为88.0%和36.4%;Sur-vivin预测放疗敏感性分子标志时的灵敏度为77.6%,特异度为42.9%,Youden指数为0.21,准确度为69.4%,而阳性预测值和阴性预测值分别为83.3%及33.3%;联合COX-2和Survivin预测鼻咽癌放疗敏感性的灵敏度为72.4%,特异度为85.7%,Youden指数为0.58,准确度为72.2%,阳性预测值和阴性预测值分别为77.8%及66.7%。结论 COX-2和Survivin可作为预测鼻咽癌放疗敏感性正相关的候选分子指标之一,联合检测COX-2和Survivin可提高预测价值。【Objective】To detect COX-2 and survivin expression and to evaluate the predicting value about radiotherapy in nasopharyngeal carcinoma.【Methods】The diagnosed nasopharyngeal carcinoma patients were collected from 2008.1 to 2010.1 as experimental group,nasopharyngitis patients were enrolled as the control group.COX-2 and survivin were detected by immunohistologic SP method in biopsy tissues of these patients,the relation between their expressions and therapy prognosis was analyzed,then the predicting value of COX-2 and survivin in radiotherapy prognosis was evaluated.【Results】The expression rate of COX-2 and survivin in the radiation sensitive and insensitive patients had significant differences(P =0.000,P =000),the evaluation outcome of COX-2 and survivin about predicting value were that sensitivity rate were 75.9% and 77.6%,specificity rate were 57.1% and 42.9%,Youden index were 0.33 and 0.21,accuracy rate were 72.2% and 69.4%,positive predictive value and negative predictive value were 88.0% and 83.3%,36.4% and 33.3%.While combined evaluated,sensitivity rate was 72.4%,specificity rate was 85.7%,Youden index was 0.58,accuracy rate was 72.2%,positive predictive value and negative predictive value were 77.8% and 66.7%.【Conclusions】COX-2 and survivin could be chosen as the effective and sensitive biomarks in radiotherapy sensitivity prediction,combined detection of COX-2 and survivin
