摘　　要：目的:分析中医医院和西医医院在费用支出上的影响因素和差异,探寻中医医院基本费用特征及分布规律,为临床路径设计提供依据。方法:采用描述统计分析方法,对北京市医保中心提供的2005—2008年北京市区进入城镇医保系统,符合本研究纳入标准的结肠癌、直肠癌患者共9 621例次进行统计性描述分析。结果:中医院收治的患者占全部患者的13.9%;中医医院平均费用低于综合医院及专科医院,有显著差异;是否伴严重合并病、是否手术、不同医院等级、是否中医院、平均住院天数等因素对费用有显著影响;而年龄、性别、入院次数、是否晚期、是否多发癌等对费用影响不显著;住院天数和伴严重合并症是影响中医院结直肠癌患者费用的主要因素。结论:结直肠癌患者在中医医院住院治疗的比例明显偏低。中医医院在治疗结直肠癌方面平均费用低于综合医院及专科医院。Objective： Analyses the influencing factors and difference on hospitalization costs of the patients from Traditional Chinese Medicine （TCM） hospital and western medicine hospital, research the basic cost characteristics of TCM hospital and the distribution law of the costs, provide the basis for design the clinic path. Methods： Descriptive statistical analysis the data of the 9 621 cases with colorectal cancer from basic medical insurance in Beijing city, from 2005 to 2008. Results： The cases from TCM hospital accounting for 13.9% of all cases. The average cost in TCM hospital was lower than the special hospital and the general hospital significantly. The following factors impacted significantly on the cost： complication with severe diseases, surgery, different grades of hospital, whether TCM hospital, the proportion of CHM usage among all medication, average hospitalization days, etc. The following factors were not significant effects on costs： age, gender, frequency of hospital admissions, whether advanced stage, whether multiple cancers, etc. The major factors that impacted on cost in TCM hospital were hospitalization days and severe complication. The proportion of more than or equal to 40 percent in TCM usage, TCM hospital was more than western medicine hospital significantly. Conclusion： The proportion of the cases from the TCM hospital was lower than other style hospital significantly. The TCM hospital had the characteristics of the average cost was low, and the proportion of TCM usage was higher.
