翼状胬肉两种术式的临床效果观察

摘　　要：目的:比较羊膜移植术和自体结膜移植术治疗翼状胬肉的临床疗效,比较两者的优缺点。方法:将362例482眼翼状胬肉随机分为A(250只眼)、B(232只眼)两组,两组于显微镜下切除胬肉组织后,A组采用羊膜移植术,B组采用自体结膜移植术,术后随访6-12个月。观察两组疗效并进行分析比较。结果:角膜创面上皮平均愈合时间,A组平均4.52天,B组平均4.25天,两组比较差异无统计学意义(P>0.05)。A组复发25只眼,复发率为10.0%;B组复发7只眼,复发率为3.02%,P<0.05;A组Ⅰ级、Ⅱ级外观175眼(70%),B组Ⅰ级、Ⅱ级外观197眼(84.91%),P<0.05,两组比较差异具统计学意义。结论:自体结膜和羊膜移植应用于翼状胬肉术中,降低手术复发率,自体结膜移植优于羊膜移植,在具体应用和病例选择上要具体对待。Objective To compare the amniotie membrane transplantation and conjunctival autograft transplantation for pteryglum treatment, and compared their advantages and disadvantages.Methods 362 cases 482 eyes with pterygium were randomly divided into A （250 eye）, B （232 eye） in two groups, After excision of pterygium microsurgery operation amniotie membrane transplantation was used in group A, au- tolagous conjunctival transplantation was used in group B, in the postoperative 6-12 months follow-up.Observation of efficacy of the two groups of comparison and analysis.Results corneal epithelial average healing time was 4.52 days in group A, and was 4.25 days in group B. No significant differences between the two groups （P〉0.05）.A group recurred in 25 eyes, the recurrence rate was 10%; B group recurred in 7 eyes, the recurrence rate was 3.02%, P〈0.05; in group A appearance as grade I, grade II was 175 eyes （70%）, in B group appearance as grade I, grade II was 197 eyes （84.91%）, P〈in0.05, the difference between the two groups was statistically significant.Conclusions conjunc- tival autografi transplantation and amniotie membrane transplantation applied in pterygium surgery operation, in reducing the recurrence rate and improve appearance, conjunctival autograft transplantation is better than amniotic membrane transplantation.
