摘 要:目的:通过多排螺旋CT血管生成、图像融合法显示卵巢肿瘤供血动脉及引流静脉的特征,提高卵巢肿瘤定位诊断的准确率。方法:回顾性分析经病理确诊的52例卵巢肿瘤,采用多排螺旋CT血管成像(MSCTA)血管生成及图像融合法,重建盆腔血管,通过显示肿瘤的供血动脉为子宫动脉卵巢支和(或)卵巢动脉,肿瘤的引流静脉为卵巢静脉丛,明确定位肿瘤起源于卵巢,并以病理结果进行对照分析。结果:MSCTA显示52例卵巢肿瘤的供血动脉46例,显示率为88.5%(46/52),根据肿瘤的供血动脉分别为子宫动脉卵巢支和(或)卵巢动脉,判断为卵巢来源肿瘤的准确率为95.7(44/46),引流的卵巢静脉(OV)丛显示例,显示率为92.3%(48/52),根据卵巢静脉丛显示并进入肿块定位诊断卵巢肿瘤准确率为95.8%(46/48)。结论:通过MSCT血管生成、图像融合法显示卵巢肿瘤供血动脉的来源、部位和数量以及引流静脉的类型和走行和分布,能明显提高卵巢肿瘤定位诊断的准确率。Purpose: To study the ovarian neoplasms feeding arteries and draining veins base on multislice spiral CT angiography (MSCTA) by postprocessing technique, such as add-vessel(AV) and image fusion technique, and to raise the accuracy of localization diagnosis. Methods: Fifty-two cases of ovarian neoplasms with pathological diagnosis were analyzed retrospectively. Ovarian vessel was reconstructed base on MSCTA. According to observe the ramus ovaricus arteriae uterinaeta and/or ovarian artery as the feeding arteries of tumor and the ovarian venous plexus as draining veins, the tumor originated from ovary was diagnosed definitely and further comparative analysis was done with pathologic findings. Results: Forty-six feeding arteries of the 52 cases of ovarian neoplasms were showed, and the displaying rate was 88.5% (46/52). The accuracy rate of diagnosing the tumor originated from the ovary based on ramus ovaricus arteriae uterinaeta and/or ovarian artery as the feeding artery was 95.7% (44/46). 48 ovarian venous plexus as the draining vein were showed, and the displaying rate was 92.3% (48/52). Localization diagnosis of the ovarian neoplasms was made based on the ovarian venous plexus displaying and the relationship with the tumor, and the accuracy rate was 95.8% (46/48). Conclusion: Base on the study of the ovarian neoplasms feeding arteries and draining veins with MSCTA and postprocessing technique, the accuracy of localization diagnosis could be improved obviously.
