摘  要:目的:探讨食管癌术后各种并发症发生的危险因素.方法:抽取自2009—07-2011-12于我院行食管癌手术患者88例作为研究对象,收集病史中的下列资料:一般情况、既往史、术前检查指标、术前用药、手术资料及围手术期化验、术后资料、术后并发症.回顾性分析食管癌术后各种并发症的发生情况及原因,单因素分析用X2检验;多因素分析采用Logistic回归.结果:术后肺部并发症27.9%,吻合口瘘2.3%,心律失常11.6%,切口感染9.3%,并发症死亡率1.2%.食管癌术后并发症是由多种因素决定的,以吻合口瘘、心律失常、肺部并发症多见,且为主要死亡原因.结论:严格选择病人、充分做好术前准备,严格规范手术操作,及时、正确的围手术期处理是降低手术并发症和死亡率的主要措施.Objective: To discuss the risk factors of complications after esophageal carcinoma operation. Methods: 88 patients who underwent esophageal carcinoma operation in our hospital form July 2009 to December 2011were analyzed, and the data of medical history were collected as follows: general situation, past history, examination indexes and medication before operation, operation data, perioperative assay and complications after operation, the occurrence condition and reasons of complications after operation were retrospectively analyzed. The risk factors were analyzed by univariate and multivariate analysis. Results: The results showed: lung complication was 27.9%, anastomotic leakage was 2.3%, arrhythmia was 11.6%, incision infection was 9.3% and the mortality of complications was 1.2%. Conclusion: The main complications after esophageal carcinoma operation are anastomotic leakage, arrhythmia and lung complication. They are also the main reasons why patients died. The best measures to reduce the complications and death are to select the patients appropriately, to prepare the operations well before the operation, to observe the operation rules strictly and to manage the perioperative care timely and correctly.
