SBK治疗高度近视眼的疗效研究

摘　　要：目的观察前弹力层下激光角膜磨镶术（sub—Bowmans keratomileusis，SBK）治疗高度近视的有效性、可预测性和安全性。方法回顾性分析自2010年5月到2011年5月行准分子激光治疗的高度近视患者80例，其中行SBK手术患者40例（80只眼），行传统LASIK手术患者40例（80只眼）。分别于术后1d、lW、2w、1m、3m随访观察视力、屈光度、角膜瓣愈合情况及角膜基质厚度。结果观察术后1d、1W、2W、1In、3m，裸眼视力≥1．0者，SBK组分别占80％、87．5％、92．5％、92．5％、92．5％；LASIK组分别占82．5％、85％、90％、97．5％、97．5％，差异无统计学意义（P〉0．05）。屈光度在±0．50D之间，SBK组分别占77．5％、82．5％、85％、85％、85％；LASIK组分别占72．5％、80％、87．5％、87．5％、87．5％，差异无统计学意义（P〉0．05）。术后各时间点角膜瓣愈合情况均良好，角膜瓣边缘贴附紧密，均未发现角膜上皮植入及HAZE现象。术后SBK组角膜基质平均厚度为（436．9±28．06）μm；LASIK组角膜基质平均厚度为（404．15±27．86）μm，差异有统计学意义（P〈O．05）。结论SBK治疗高度近视的有效性和可预测性与LASIK相同，安全性明显优于LASIK。SBK术后角膜基质保留更多，扩大近视的矫治范围，有助于高度近视或（和）角膜薄的患者以及二次手术矫治屈光同退提供机会。术后角膜生物力学稳定性更好。Objective To explore the clinical effect of sub-Bowmans keratomileusis on moderate to high myopia and evaluate the efficiency, predictability and security on SBK. Methods The refrac- tive and visual outcomes of 80 cases （160 eyes） with high myopia that underwent laser in situ ker- atomileusis from 2010-05 to 2011-05 were analyzed retrospectively, in which the SBK group had 40 cases （80 eyes） while the LASIK group had 40 cases （80 eyes）. The naked vision, refraction status, corneal healing conditions and stromal bed thickness were recorded and compared at postoperative 1st day, 1st week, 2nd week, 1st month and 3rd month. Results At postoperative 1st day, 1st week, 2nd week, 1st month and 3rd month the percentages of naked vision better than 1.0 were 80%, 87.5%, 92.5%, 92.5%, 92.5% in SBK group, were 82.5%, 85%, 90%, 97.5%, 97.5% in LSIK group, no statistical difference was found （P 〉0.05）, The percentages of diopter between -0.5 D and 0.5 D were 77.5%, 82.5%, 85% , 85%, 85% in SBK group, were 72.5% , 80%, 87.5% , 87,5% , 87.5% in LSIK group, no statistical difference was found （P 〉0.05）. The corneal flaps heal condi- tion were good, the corneal flaps＇ edge attached with close and there were no corneal epithelium im- plant and HAZE. Stromai bed thickness were （436.9±28.06）μm in SBK group and （404.15±27.86）μm in LASIK group, there was significant difference between the two groups （P 〈0.05）. Conclusions SBK has the similar efficacy and predictability with LASIK. But the safepy and comeal biomechani-cal stability at postoperative of SBK are superior to those of LASIK. SBK has more corneal stroma and expanding the scope of myopia treatment..
