眼眶静脉性血管瘤100例CT表现

摘　　要：目的分析眼眶静脉性血管瘤的cT表现，提示该病的特殊cT征，作为诊断依据。方法复习经手术及病理组织学证实的静脉性血管瘤CT片共100例，分析其所在的位置、范围、形状、边界、密度、CT值及继发性改变，并结合临床表现进行综合分析。结果100例病变均被CT发现。内上象限47例，外上象限11例，内下象限10例，外下象限7例，眶内侧7例，正上方7例，正下方5例，球后2例，占满全眶4例；不规则形90例，分叶形6例，类圆形3例，长椭圆形1例；71例为不均质，29例为均质；其中有静脉石28例。眶骨骨变薄或凹陷者16例，眶腔扩大50例，筛窦变窄16例，眶上裂扩大2例，眶下裂扩大1例。结论CT能够在术前对静脉性血管瘤作准确定性、定位诊断，对制定手术方案及其他治疗方案有很大帮助。Objective To study the appearance of orbital Venous hemangioma on CT, then to re- veal the CT characteristics of the disease for diagnosis. Methods The CT images of 100 cases were reviewed, and all had proved by histopathology or operation. And the position, extent, shape, boundary, density, CT value, subsequent changes, and combination with clinical manifestation were analyzed. Results CT could discover all the lesions. There were 47 cases in the upper inner quad- rant, ll cases in the upper outer quadrant, 10 cases in the lower inner quadrant, 7 cases in the low- er outer quadrant, 7 cases in the inside of orbital, 7 cases in the inferior aspect, 5 cases in the supe- rior aspect, 2 cases behind the eyeball, 4 cases in the all orbit. The shape of lesions were irregulari- ty 90 cases, sublobe 6 cases, class round 3 cases, oval 1 case. The density of the lesions was het- erogeneous 71 cases, homogeneous 29 cases, included 28 cases calcific plaque （28%）. The bone wall thinning or sag were 16 cases, orbitofrontal cavity expansion was 50 cases, ethmoidalsinus narrowing 16 cases, superior orbital fissure expanding was 2 cases, inferior orbital fissure expanding was l case. Conclusions Orbital CT scanning is the effective way in the preoperative diagnosis of orbital Venous hemangioma for determining its nature and location. It is helpful to formulating the proce- dures of operation.
