正常人、尿毒症和血液透析患者大颗粒淋巴细胞的研究

摘  要:大颗粒淋巴细胞(Large Granular Lymphocyte,LGL)与自然杀伤细胞(Natural Killer,NK)关系密切,有人认为LGL反应NK细胞总数。本研究测定了55例正常人、16例尿毒症和34例透析尿毒症患者外周血大颗粒淋巴细胞百分率,研究了正常人、尿毒症患者LGL的性别和年龄差别以及血液透析对其影响。The peripheral blood LGL rates were determined in 55 normal subjects, 16 uremic and 34 hemodialyzed patients. The findings were:(1) Total LGL rate was 23.37±1.19 in normal males, 15.43±1.15 in normal females and the rate of LGL with more than 6 azurophilic granules (6 granular LGL) was 17.33±1.00 and 11.59±1.05 in normal males and females respectively, Total LGL rates in the male were higher than those in females. (2) No age difference of LGL rate was found in normal males. (3) LGL rate in the uremic palients was significantly lower than that in the mormal subjects and, just contrarily, it was greater in females than that in males. (4)The LGL rate in hemodialyzed patients was also significantly lower than that in normal subjects and it was greater in females than that in males too. However, the effect of different periods on the LGL rate was foundin the term analysis. After 4-month term hemodialysis, the LGL and≥6 granular LGL rate was increased significantly and the same trend appeared in females. No significant difference was found between predialysis and postdial6ysis counts of LGL rates. (5) There was a tendency that total LGL and≥6 gtaniulated rates were negatively correlated with serum creatinine (SCr) values. Basing on the findings above, we conclude that the LGL rates in uremic and in henodialyzed patients are lower than thos in normal aubjects, although more than 4-months dialysis seems beneficially make LGL rate a little bit higher than that at the beginning of dialysis. The posibility of occurrence of viral infections or tumor should be always considered in uremic patients, even in hemodialysed patients.
