摘 要:HALO系统分步环周和局灶性射频消融术是一种应用前景广阔的内镜治疗Barrett食管(BE)的新技术。环周消融通过球囊辅助的双极性电极完成,局灶性消融则由内镜头端帽状结构的分节状电极完成。研究表明射频消融术可安全、有效地治疗不伴或伴有异型增生的BE并可克服其他消融术的诸多不足。本文就射频消融术的设备与操作方法、临床应用、疗效、安全性以及并发症等作一综述。Stepwise circumferential and focal radiofrequency ablation using the HALO system is a novel and promising endoscopic ablation modality for Barrett' s esophagus (BE). Circumferential ablation is performed using a balloon-based bipolar electrode, while focal radiofrequency ablation is performed using an endoscope-mounted bipolar electrode on an articulated platform. Recent studies have shown radiofrequency ablation to be effective and safe for the treatment of BE with or without dysplasia. The technology appears to overcome many limitations of prior endoscopic ablation modalities. This article reviewed the device and technique, and the clinical application, efficacy, safety and complications of radiofrequency ablation for the treatment of BE.
