保留脾脏的全胰十二指肠联合切除、区域淋巴结清扫术20例疗效评价

摘　　要：目的对保留脾脏的全胰十二指肠联合切除、区域淋巴结清扫术的疗效进行评价。方法对2008年6月～2011年6月20例接受保留脾脏的全胰十二指肠联合切除、区域淋巴结清扫术的患者的临床资料进行回顾性分析。结果 20例胰头癌患者完成手术,无术中死亡病例,4例(20.0%)发生术后并发症,1例(5.0%)术后60天内死亡。术后血糖均能较好的控制,生活质量较术前明显提高。20例均获得随访,中位生存期16.2个月(1.5～20)。结论保留脾脏的全胰十二指肠联合切除、区域淋巴结清扫术虽然对患者远期生存率无明显改善,但在减少了患者术后并发症的基础上明显改善了患者的生存质量。Objective To evaluate the clinical outcome of spleen-preserving total pancreatectomy combined duodenectomy,regional lymph node dissections.Methods The clinical data of 20cases of spleen-preserving total pancreatectomy combined duodenectomy,regional lymph node dissections from june 2008 to June 2011 was retrospectively analyzed.Results 20 cases of pancreatic head carcinoma patients were accepted spleen-preserving total pancreatectomy combined duodenectomy,regional lymph node dissections.There was no intraoperatie deaths.Complications occurred in 4cases（20.0%） post the operation and 1cases（5.0%） died in 60 days after the operation.Boold glucose was maintained normal effectively after the operation.Twenty cases were followed-up and median survival was 16.2 months（1.5-20 months）.Conclusion Although spleen-preserving total pancreatectomy combined duodenectomy,regional lymph node dissections cannot improve patients＇ long-term survival rate,but it can reduce the postoperative complications and improve the quality of life of patients.
