晚期癌痛皮下自控镇痛效果分析

摘　　要：目的探究在晚期癌症患者中应用皮下自控法行癌痛治疗的临床效果。方法选择笔者所在医院2009年3月～2012年1月收治的82例患者,按随机数字表分为皮下自控镇痛组(皮下组)及传统治疗镇痛组(传统组),皮下组将吗啡(40mg)及咪唑安定(20mg)混合配置为50mL药液,用自控泵皮下注入镇痛;传统组予吗啡(0.1～0.2mg/kg)及安定(20mg)肌注镇痛。结果皮下组对比传统组,第1、7、14天镇痛评分均较低,差异有统计学意义(P<0.05);在镇痛治疗满意度方面:皮下组满意率较传统组高,不合格率较传统组低,差异均有统计学意义(P<0.05)。结论 PCSA通过持续小剂量给药,一方面药物应用剂量小,可最大程度避免药物所产生的不良反应;另一方面血药浓度稳定,患者或家属可自行调控,有效满足治疗个体化需求,在晚期癌痛患者中应用PCSA简便易行,临床效果理想。Objective To investigate the clinical therapeutic effect of controlled subcutaneous method for patients with advanced cancer.Methods 82 patients with advanced cancer in our hospital from March 2009 to January 2012 were randomly divided into subcutaneous analgesia group （subcutaneous group） and the traditional treatment analgesia group （traditional group）.Subcutaneous group received morphine （40mg） and midazolam （20mg）,which were hybrid configured to 50 mL liquid medicine for control pump subcutaneous injection;traditional group received morphine （0.1-0.2mg/kg） and diazepam （20mg） by intramuscular injection.Results Analgesic scores of the first day,the seventh day and the fourteenth day of subcutaneous group were lower than the traditional group,the differences were statistically significant （P 0.05）;In the analgesia treatment satisfaction aspects：satisfaction rate of subcutaneous group was higher than traditional group,fraction defective was lower than traditional group,the differences were statistically significant（P 0.05）.Conclusion PCSA through continuous small dosage of medicine,on the one hand,avoid drug adverse reactio by the greatest extent;On the other hand,blood concentration of patients were stability,the patients or family members can control it by themselves,effectively meet the individual needs,so PCSA is simple,convenient and feasible for patients with terminal cancer pain.
