术中结肠灌洗在梗阻性左半结肠癌中的临床应用

摘　　要：目的左半结肠癌并急性肠梗阻因无法进行充分的肠道准备,常需行分期手术。本研究探讨术中结肠灌洗后一期切除吻合的方法在梗阻性左半结肠癌治疗中的临床疗效。方法回顾性分析漯河市中心医院2006年10月-2011年10月收治的28例梗阻性左半结肠癌行术中结肠灌洗后行一期吻合病例的资料。结果 28例梗阻性左半结肠癌患者均采用术中结肠灌洗后行一期切除吻合,治愈27例,治愈率96.4%,死亡1例,病死率3.6%,并发症发生4例,发生率14.3%。术后随访23例,失访5例,其中5年生存人数8例,3年以上10例。结论术中结肠灌洗一期切除吻合是梗阻性左半结肠癌的一种理想术式,适合于年龄在80岁以下,无明显脏器功能障碍的患者,且应在术前全面评估患者的情况下实施。Objective Staged-operation was performed in left-sided malignant colonic obstruction patients due to not adequate bowel preparation. This study was to investigate the clinical effect of one-stage resection-anastomosis procedure in the treatment of left-sided malignant colonic obstruction. Methods Twenty-eight left-sided malignant colonic obstruction patients were reviewed retrospectively between October 2006 and October 2011. All cases were treated by one-stage resection-anastomosis procedure. Re- suits 27 cases were healed by one-stage resection-anastomosis procedure in the 28 patients of left-sided malignant colonic ob- struction. The cure rate was 96.4% , the postoperative complications was occurred in 4 cases （4/28,14.3%） and 1 case died,the mortality was 3.6%. Post-operative follow-up of 23 cases, and cases were lost, the number of 5-year survival was 8 cases, 10 cases more than three years. Conclusion The procedure for one-stage resection-anastomosis in the treatment of left-sided malignant co- lonic obstruction was an ideal technique. It was suggested that the procedure was suitable for patients under 80 years old without significant organ dysfunction. The patient＇ s condition should be assessed before operation comprehensively.
