摘 要:目的检测口底鳞癌组织中表皮生长因子受体(EGFR)和环氧化物酶2(COX-2)蛋白的表达,探讨其相关的临床意义。方法收集2009年1月-2011年12月口底癌手术切除标本48例,所有病例均经病理确诊,另取癌旁非癌组织作为对照。病理分化程度、有无淋巴结转移、临床TNM分期分组对照EGFR和COX-2蛋白的表达。免疫组化检测组织中EGFR和COX-2蛋白的表达。结果 EGFR在口底鳞癌标本中的阳性率为75.0%,显著高于周围正常对照组织的29.5%(P<0.01),COX-2在口底鳞癌标本中的阳性率为81.3%,显著高于周围正常对照组织的35.4%(P<0.01)。图像分析显示EGFR和COX-2的相应光密度(IOD)值在口底癌组分别为126.45±41.33和144.76±48.59,显著高于癌旁对照组织的61.22±21.42和74.56±23.12(P<0.01)。EGFR和COX-2蛋白的表达量在口底鳞癌的分化程度、有无淋巴结转移、临床不同分期之间差异有统计学意义(P<0.05)。相关性分析显示口底鳞癌组织中EGFR和COX-2基因的表达呈正相关(r=0.736,P<0.01)。结论 EGFR和COX-2在口底鳞癌的发生发展中起到一定的作用,联合检测对于口底鳞癌的诊断、预后评估及治疗具有一定的临床指导意义。Objective To investigate the protein expression of EGFR and COX-2 in human squamous cell carcinoma of the floor of mouth and discuss its clinical significance. Methods Total 48 patients with floor-of-mouth cancer were enrolled in our study. The adjacent normal floor-of-mouth tissues were used as a control. The histological grade,lymphatic metastasis and clinical patho- logic stage were recorded. The protein expression of EGFR and COX-2 was detected by immunohistochemical technique. Results The positive expression rates of EGFR and COX-2 were 75.0 and 81.3% in floor-of-mouth cancer, respectively. How- ever, in adjacent normal tissues only 29.5 and 35.4% of EGFR and COX-2 can be detected. There was significant difference be- tween the two groups(P 〈 0.01 ). The IOD of EGFR ( 126.45 ± 41.33 VS 61.22 ± 21.42) and COX-2 ( 144.76 + 48.59 VS 74.56 + 23.12) was higher in cancer tissues than that in normal tissues ( P 〈 0.01 ). The expression of EGFR and COX-2 was significant related with histological grade, lymphatic metastasis ,and clinical pathologic stage in human floor-of-mouth cancer( P 〈 0.05 ). The protein expression of EGFR was significantly positive related with COX-2 in floor-of-mouth cancer patients ( r = 0. 736, P 〈 0.01 ). Conclusion The over-expression of EGFR and COX-2 had a close relationship with the development of floor- of-mouth cancer. The high-level expression of EGFR and COX-2 might be used as the marks for estimating the degree of differen- tiation of floor-of-mouth cancer.
