改良额底入路切除垂体瘤术中垂体柄保护探讨

摘　　要：目的探讨在改良额底人路方式下，开颅垂体瘤切除术中垂体柄的保护策略。方法选择在改良额底人路方式下切除垂体瘤50例。结果50例肿瘤全切45例，次全切5例。48例肿瘤包膜外垂体柄均得以完整保留，1例未探查到垂体柄，1例辨认不清。结论采取改良额底入路方式，垂体柄保护好，对视力影响较小。Objective To explore the microsurgical resection of pituitary adenoma and the technique to preserve the pituitary stalk via modified frontalbase approach. Methods Fifty cases were treated via modified frontalbase approach. Results Total removal of the tumor was got in 45 cases （90%） and subtotal resection was performed in 5 cases（ 10% ）. The pituitary stalks were reserved in 48 cases（96% ）. We couldn＇t find the pituitary stalks in 2 cases. The pituitary stalks can be protected well via modified frontalbase approach and can reduce the postoperative incidence of adverse reactions. Conclusion The protection of pituitary stalk is effective via modified frontal base approach with a limited effect on eyesight.
