摘  要:目的探讨艾司西酞普兰和文拉法辛治疗抑郁障碍的效果。方法将123例抑郁障碍患者随机分为研究组(62例)和对照组(61例),研究组给予艾司西酞普兰系统治疗,对照组给予文拉法辛系统治疗,疗程均为4周。入组患者应用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)与不良反应量表(TESS)在基线及治疗后第1、2、4周末分别评定疗效与不良反应。结果与基线相比,研究组和对照组患者的HAMD及HAMA评分在第1周末即有显著性降低,差异有显著性意义(P<0.05),而在第1周末研究组患者的HAMD评分显著低于对照组患者(P<0.05)。在治疗后的第4周末,对照组患者的HAMA评分显著低于研究组(P<0.05)。两组患者的临床整体疗效相当(P>0.05)。两组均未出现严重不良反应。结论艾司西酞普兰和文拉法辛均可有效、安全地改善抑郁障碍患者的症状,而艾司西酞普兰改善抑郁症状起效更快,文拉法辛改善焦虑症状效果更好。Objective To explore the efficacy of escitalopram vs. venlafaxine in improving symptoms of depression. Methods 123 patients with depression were randomly divided into study group (62 cases) treated with escitalopram and control group (61 cases) treated with venlafaxine for 4 weeks. All patients were assessed with Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA) and Treatment Emergent Symptom Scale (TESS) at baseline and the 1 st, 2nd, 4th weekend of treatment. Results Scores of HAMD and HAMA in both groups at the 1 st weekend decreased significantly compared with the baseline (P 〈 0.05 ). Score of HAMD in study group were significantly lower than that in control group at the 1 st weekend (P 〈0.05 ). Score of HAMA in control group were significantly lower than that in study group at the end of the treatment HH (P 〈 0.05 ). The total efficacy in both groups were equivalent (P 〉 O. 05 ). There was no severe side effect in the two groups (P 〉 O. 05). Conclusion Both escitalopram and venlafaxine is effective and safe in the treatment of depression. Escitalopram takes effect more faster in improving depressive symptoms, venlafaxine is more effective in improving anxiety symptoms.
