个体化干预对舌下含服脱敏治疗儿童依从性的影响

摘　　要：目的观察个体化干预对变态反应病儿童持续舌下含服粉尘螨滴剂特异性免疫治疗依从性的影响。方法变态反应疾病患儿300例按出生单双日期分成无干预对照组(单日出生)及干预组(双日出生)各150例。两组患者在接受脱敏治疗后均建立脱敏档案及告知脱敏常识。对照组未予任何干预,干预组给予知识认知、心理、行为等干预措施。观察两组患者治疗的依从性。结果哮喘患者停止吸入激素和喘息急性发作例次、患者自我症状的改善评价等指标,干预组优于对照组(P<0.01);平均持续脱敏时间干预组明显长于对照组(P<0.01);干预组中60.7%的患者坚持脱敏治疗2年以上,对照组中无1例患者坚持脱敏治疗至2年。结论个体化的干预能显著提高患者舌下脱敏治疗的依从性,进而提高治疗效果。Objective To investigate the effect of personal intervention on the compliance with sublingual dust mite drops specific immunotherapy in children with allergic diseases. Methods Three hundred patients were divided into two groups according to whether they birth on odd-numbered day（ control group, 150 cases） or even-numbered day （treatment group, 150 cases）. Patients in the two groups were treated with immunotherapy, and were awareness of knowledge about desensitization after treatment. Their desensitization documents were also established. No intervention was performed in the controls, while personal intervention on the patients＇ cognition, psychology and behavior was performed in the treatment group. The compliance of all the children was observed. Results The treatment group was superior to the control group in the indices such as frequency of stopping inhaling corticosteroid, acute asthma attacks, and the improvement of patients＇ was longer in the treatment group group had taken the sublingual symptoms （ P 〈 0.01 ）. The average duration of patients undergoing immunotherapy than that in the control group（ P 〈 0.01 ）. 60.7% of the patients in the treatment immunotherapy for more than 2 years, while no one received the immunotherapy treatment for 2 years in the control group. Conclusion Personal intervention can significantly improve the compliance with sublingual immunotherapy in patients,which helps to improve the efficacy of the treatment.
