食管癌发展过程中COX-2蛋白和p53蛋白表达的研究

摘　　要：目的观察环氧合酶-2(COX-2)蛋白和p53蛋白在食管癌及癌前病变组织中的表达情况。方法应用免疫组化方法检测43例鳞状细胞癌及22例增生性病变和6例正常食管黏膜鳞状上皮中COX-2蛋白和p53蛋白的表达情况。结果正常食管黏膜上皮、单纯性增生、轻/中度不典型增生中未发现COX-2蛋白及p53蛋白表达。COX-2蛋白在重度不典型增生、食管鳞状细胞癌中表达阳性率分别为25%和74%,p53蛋白分别为75%和56%。COX-2蛋白、p53蛋白强阳性表达率在Ⅰ～Ⅱ期食管癌中分别为40%和18%,Ⅲ～Ⅳ期分别为为73%和77%。COX-2蛋白、p53蛋白表达与淋巴转移、浸润深度、肿物大小无关。结论 COX-2蛋白和p53蛋白的表达与食管癌的发展关系密切,是食管癌发展过程中的一个重要监测指标,可作为食管癌可疑病例定期随访的指标之一。Objective It is to observe the expressions of cyclooxygenase-2（COX-2） protein and p53 protein in the tissue of esophageal cancer and precancerous lesions.Methods Forty-three biopsy specimens from patients with esophageal squamous-cell carcinomas,twenty-two from patients with esophageal proliferative lesion,and six from individuals with normal esophageal mucosa were detected for COX-2 protein and p53 protein expression by immunohistochemistry.Results The expression of COX-2 protein and p53 protein was not observed in normal esophageal epithelium mucosae,simplex hyperplasia and atypical hyperplasia from mild to moderate.The positive rate of COX-2 protein expression was 25% in severe atypical hyperplasia and 74% in esophageal spuamous-cell carcinomas,and the positive rate of p53 protein expression was 75% and 56% respectively.The strong positive rate of COX-2 protein and p53 protein expression was 40% and 18% in I to II stage esophageal spuamous-cell carcinomas and was 73% and 77% in III to IV stage respectively.The expression of COX-2 protein and p53 protein was irrelevant to Lymphatic Metastasis,infiltrate depth and bump size.Conclusion The expression of COX-2 protein and p53 protein is associated with the development of the esophageal carcinomas;it is an important index for monitoring of the development.
