彩超监测肠系膜上动脉流速对大鼠粘连性肠梗阻的诊断及茴香枳术汤疗效的判定

摘　　要：目的:探讨彩超监测肠系膜上动脉流速对粘连性肠梗阻的诊断及药物疗效的判定价值。方法:72只SPF级(雌雄各半)Wistar大鼠,随机分为正常组、模型组、大承气汤(DS)组、茴香枳术汤(HX-S)低剂量组、中剂量组和高剂量组,除正常组,其余组大鼠按照浆膜剥脱法复制大鼠粘连性肠梗阻模型,造模成功后各组分别检测肠系膜上动脉流速及肠梗阻程度,并对其关系进行统计学分析;各组分别给予生理盐水、DS、HX-S低、中、高剂量灌胃,治疗后第3、7天分别应用彩色多普勒观察大鼠肠系膜上动脉峰值流速。结果:模型组中肠系膜上动脉(superior mesenteric artery,SMA)收缩期峰值流速与肠梗阻严重程度呈正相关(r=0.418,P<0.01);治疗后第3、7天,DS组、HX-S低、中、高剂量组与模型组比较,肠系膜上动脉收缩期峰值流速均明显下降(P<0.05),各治疗组比较,HX-S高剂量组效果较显著。结论:应用高频彩超监测肠系膜上动脉流速对大鼠粘连性肠梗阻的诊断及药物疗效的判定有一定的价值。Objective： To explore the diagnosis of superior mesenteric artery（SMA） for adhesive ileus and the value of medicine.Method： Seventy-two rats were randomly allocated to normal group,model group,DaChengQiTang group（DS）,low,moderate and high dosage groups of HuiXiang ZhiZhuTang（HX-S）.Except normal group,the rats in other groups were successfully established the models with membrane stripping method and then SMA flow velocity,the degrees of intestinal obstruction and their relationship were analyzed statistically;meanwhile,the rats in six groups received intragastric administration of physiological saline,DS,low,moderate and high dosage of HX-S. At the third and seventh day after treating,SMA flow velocity was observed with color Doppler ultrasound.Result： SMA flow velocity was positively related to the degree of intestinal obstruction in model group（r=0.418,P〈0.01）; SMA flow velocity decreased obviously at the third and seventh day after treating when DS group,low,moderate and high dosage groups of HX-S were compared with model group（P〈0.05）.High dosage group of HX-S obtained remarkable effects when different groups were compared.Conclusion： High-frequency color Doppler ultrasound monitoring SMA flow velocity manifests certain value for the diagnosis of adhesive ileus in rats and the effects of medicine.
