利培酮联合文拉法辛对精神分裂症患者临床症状改善及社会功能恢复的观察

摘　　要：【目的】探讨利培酮联合文拉法辛治疗精神分裂症患者临床症状和社会功能的疗效及安全性。【方法】将80例精神分裂症患者随机分为利培酮＋文拉法辛组（研究组）和利培酮组（对照组）各40例，利培酮治疗剂量4～6mg／d，文拉法新治疗剂量150mg／d。治疗时间为12周，分别于治疗前、治疗4周、8周、12周末使用阳性与阴性症状量表（PAN＆S）、精神病患者社会功能评定量表（PSP）评估临床疗效，锥体外系反应量表（RSESE）评估治疗副反应。血常规、血生化及心电图检查来评估药物的安全性。【结果】治疗12周末，两组间PANSS总分、阳性因子分、阴性因子分、PSP总分差异均有统计学意义（P〈0．05或P〈0．01）。两组不良反应多为轻度，最常见不良反应均为锥体外系反应。【结论】利培酮合用文拉法新治疗精神分裂症患者临床症状及社会功能的疗效优于单用利培酮治疗，且安全性较好，未发生严重的不良反应。[Objective] To explore the efficacy and safety of risperidone combined with venlafaxine for the treatment of clinical symptoms and social function in schizophrenic patients. [Methods]Totally 80 patients with schizophrenia were randomly assigned to ris- peridone＋venlafaxine group（study group） and risperidone group（control group） with 40 cases in each group. The dose of risperidone was 4-6mg/d and that of venlafaxine was 150mg/d. The treatment course was 12 weeks. Positive and negative syndrome scale（PAN- SS） and psychotic social function rating scale（PSP） before treatment, 4 and 8 weeks and the end of 12 weeks after treatment were used to assess clinical efficacy, while rating scale of extrapyramidal system reaction（RSESE） was used to evaluate the side-effects. Routine blood test, blood biochemistry and electrocardiogram（ECG） examinations were used to assess drug safety. [Results]At the end of 12 weeks, there were significant differences in the total score of PANSS, positive factor scores, negative factor score and the total score of PSP between two groups（ P 〈0.05 or P 〈0.01）. The adverse reactions of two groups were mild. The most common adverse reaction was extrapyramidal response. [Conclusion] The efficacy of risperidone combined with venlafaxine for the treatment of clinical symptoms and social function of Schizophrenic patients is better than simple risperidone. It is safe and has no serious adverse reactions.
