痘疹样胃炎临床病理学特点及内镜下氢离子凝固术治疗分析

摘　　要：目的：研究痘疹样胃炎的临床病理学特点,探讨根除幽门螺杆菌（HP）联合内镜下氩离子凝固术（APC）治疗痘疹样胃炎的疗效。方法：回顾性分析418例痘疹样胃炎患者临床资料与胃活检标本病理结果。将94例痘疹样胃炎患者随机分为观察组和对照组,对照组44例,观察组50例。对照组予单纯根除HP治疗,观察组加用APC治疗,1~3个月后复查胃镜。结果：痘疹样胃炎具有特殊的内镜和病理学表现,加用APC治疗可增加根除HP对痘疹样胃炎的疗效（χ2=4.005,P=0.045）。结论：我们应加强对痘疹样胃炎患者的治疗和随访,根除HP联合APC治疗痘疹样胃炎安全有效,具有重要的临床意义。Objective：To investigate the clinicopathological feature of patients with varioliform gastritis and the therapeutic effect of argon plasma coagulation therapy for varioliform gastritis.Methods： The clinical data and gastric biopsy specimens from 418 patients were retrospectively examined.The 94 cases with varioliform gastritis were divided into the observing group and the control group.The two groups received HP eradication treatment,and the observing group received APC therapy more,then another gastroscopy was followed in 1 to 3 months.Results： There were especial endoscopic and the pathologic manifestations of varioliform gastritis.The effect of HP eradication added with APC was superior to that of HP eradication alone in varioliform gastritis（χ2=4.005,P=0.045）.Conclusions： Patients with varioliform gastritis should be treat and follow up seriously.APC therapy is safe and effective for patients with refractory varioliform gastritis,which may have an important clinical significance.
