摘　　要：目的:评价前循环动脉瘤破裂早期手术的治疗效果。方法:对自发性蛛网膜下腔出血(SAH)的病人尽快行头颈CTA检查,确诊为前循环动脉瘤后3 d内开颅夹闭动脉瘤,根据病人病情及术中情况确定是否去骨瓣减压。结果:术前病人Hunt-Hess分级:Ⅱ级15例,Ⅲ级8例,Ⅳ级5例,Ⅴ级3例。术后恢复良好23例,轻-中度残5例,重残1例,死亡2例,本组总死亡率为6.45%。结论:对于前循环动脉瘤破裂的病人,尤其是合并有颅内血肿的危重病人,早期开颅夹闭动脉瘤,能减少再出血,降低死亡率。Objective：To evaluate the clinical effect of early clipping for ruptured intracranial anterior circulating aneurysm.Methods：All patients with spontaneous subarachnoid hemorrhage（SHA） received head CTA examination as soon as possible.The craniotomy and clipping of the aneurysm is carried within 3 days,under the condition of patients and intraoperative conditions determine whether decompressive craniectomy.Results：Classification of Hunt-Hess：II 15 cases,III 8 cases,IV 5 cases,V 3 cases.Postoperative recovery was good in 23 cases,mild or moderate disability in 5 cases,severe disability 1 cases,2 cases died,the mortality was 6.45%.Conclusion：For ruptured anterior circulation aneurysms patients,especially in critically ill patients with intracranial hematoma,early craniotomy and clipping of the aneurysm can reduce the recurrence of hemorrhage and mortality.
