盐酸帕洛诺司琼预防晚期胃癌化疗所致呕吐的临床研究

摘　　要：目的探讨盐酸帕洛诺司琼和盐酸托烷司琼预防晚期胃癌因全身化疗引起呕吐的疗效和安全性。方法采用随机对照双盲设计方法,选择60例含铂类和(或)5-Fu方案全身化疗的晚期胃癌患者,分别采用盐酸帕洛诺司琼和盐酸托烷司琼止吐,化疗后5d内观察患者呕吐状况及不良反应。结果化疗后24h内,盐酸帕洛诺司琼组0度呕吐为21例,盐酸托烷司琼组0度呕吐为13例。24～120h内,盐酸帕洛诺司琼组0度呕吐为18例,盐酸托烷司琼组0度呕吐为10例。使用盐酸帕洛诺司琼与盐酸托烷司琼后预防急性及延迟性呕吐的疗效比较差异均有统计学意义(P<0.05),而其不良反应比较差异无统计学意义。结论盐酸帕洛诺司琼预防晚期胃癌化疗所致呕吐的疗效优于盐酸托烷司琼,其不良反应发生率低,安全性好。Objective To observe the efficacy and safety of palonosetron hydrochloride and tropise- tron hydrochloride to prevent the vomiting caused by chemotherapy for advanced gastric cancer. Methods 60 patients with advanced gastric cancer of chemotherapy including platinum and（or） 5-Fu were enrolled in this trial with a randomized, controlled and double-blind method. The patients were randomized into two groups treated with palonosetron hydroehloride or tropisetron hydrochloride respectively. The situation of vomiting and adverse reactions was observed in 5 days after chemotherapy. Results In 24 hours, there were 21 ea- ses with grade 0 vomiting in the palonosetron hydrochloride group, while there were 13 cases with grade 0 vomiting in the tropisetron hydrochloride group;in 24-120 hours, there were 18 cases with grade 0 vomiting in the palonosetron hydroehloride group, while there were 10 eases with grade 0 vomiting in the tropisetron hydrochlorlde group. Palonosetron hydrochlorlde was superior to troplsetron hydrochlorlde in preventing a- cute or delayed vomiting（P 〈 0. 05 ）. There was no significant difference in the adverse reactions caused between palonosetron hydroehloride and tropisetron hydroehloride. Conclusions Palonosetron hydroehlo- ride was superior to tropisetron hydrochloride in preventing vomiting caused by chemotherapy for advanced gastric cancer and it was safe with few adverse reactions.
