Vater壶腹部胆、胰管损伤诊治体会

摘　　要：目的探讨Mater壶腹部胆、胰管损伤的诊治策略。方法总结9例Vater壶腹部胆、胰管损伤者的临床资料。结果均及时诊治，内镜下十二指肠乳头切开取石后Mater壶腹部胆、胰管损伤4例，开腹胆管探查术时Bakes探条粗暴操作损伤5例。痊愈8例，死亡1例。结论预防发生Vater壶腹部胆、胰管损伤至关重要；及时诊断、准确判断是关键，合理选择治疗手段是难点。Objective To investigate the diagnosis and treatment strategies of iatrogenic injury in choledocho-panereatico-duodenal junction. Methods The clinical data of 9 patients in our hospital with iatrogenic injury in choledocho-pancreatico-duodenal junction were analyzed. Results All patients were diagnosed and treated timely. Iatrogenic injury in choledoeho-pancreatieo-duodenal junction oc- curred in 4 cases after stone removal by endoscopic sphincterotomy, and in 5 cases due to rough han- dling in the operation of bile duct exploration surgery using Bakes bougie. Eight cases were cured, and there was one death. Conclusion It is essential to prevent iatrogenic injury in choledoeho-pancreatico- duodenal junction. Timely diagnosis and accurate judgment are the key and it is difficult to give a rea- sonable choice of treatment.
