摘 要:目的:探讨产前超声诊断单脐动脉与胎儿先天异常之间可能存在的联系,为产前咨询和临床处理提供依据。方法:在2007年1月1日~2010年6月30日没经过筛选的52 760例孕妇中进行系统的产前超声检查,检测单脐动脉的发生情况、类型及与胎儿畸形的关系,回顾分析合并畸形种类以及与染色体异常的相关性。结果:52 760例孕妇中,检出单脐动脉208例(0.39%),其中孤立性单脐动脉120例(57.69%),伴发胎儿畸形88例(42.31%),其中22.7%为心血管系统异常,21.6%为中枢神经系统异常,20.5%为泌尿系统异常,12.5%为消化系统异常。1/3的病例合并两种或两种以上畸形。65例(31.25%)左脐动脉缺失,16例(24.61%)伴发畸形;143例(68.75%)右脐动脉缺失,72例(50.35%)伴发畸形。孤立性单脐动脉接受染色体检查的43例中,1例24岁孕妇为18-三体综合征,1例35岁孕妇为21-三体综合征,1例为染色体多态性(46,XY,9qh+)。在单脐动脉伴发胎儿畸形接受染色体检查的33例中,发现7例为18-三体综合征,1例为47,XX,+M[4]。结论:42.31%的单脐动脉合并其他畸形,心血管畸形为最多,右脐动脉缺失型多于左侧,且伴发畸形的发生率较左侧高,合并畸形的胎儿染色体异常可能性高。因此,单脐动脉胎儿行胎儿超声心动图及染色体检查是必要的。Objective:To explore the possible relationship between single umbilical artery diagnosed by ultrasonography and fetal congenital anomaly,provide a basis for prenatal consultation and clinical treatment. Methods:Systematical ultrasonography was conducted in 52 760 pregnant women who didn't receive screening from January 1st,2007 to June 30th,2010.The prevalence and types of single umbilical artery were detected,and the relationship between single umbilical artery and fetal congenital anomaly were detected,the types of combined anomalies,the correlation between single umbilical artery and chromosomal abnormality were analyzed retrospectively. Results:Among 52 760 pregnant women,208 pregnant women(0.39%) were found with single umbilical artery,and 120 pregnant women(57.69%) were found with isolated single umbilical artery,88 pregnant women(42.31%) were found with fetal congenital anomaly,including 22.7% of them with cardiovascular anomalies,21.6% of them with central nervous system anomalies,20.5% of them with urinary system anomalies,and 12.5% of them with digestive system anomalies.One third of the pregnant women were found with two kinds or more than two kinds of anomalies.A total of 65 pregnant women(31.25%) were found with absent left umbilical artery,16 pregnant women(24.61%) were found with anomalies;143 pregnant women(68.75%) were found with absent right umbilical artery,72 pregnant women(50.35%) were found with anomalies.Among 43 pregnant women with isolated single umbilical artery after chromosomal examination,one 24-year-old pregnant woman was found with trisomy 18,one 35-year-old pregnant woman was found with trisomy 21,and one pregnant women was found with chromosomal polymorphism(46,XY,9qh+).Among 33 pregnant women with single umbilical artery combined with fetal congenital anomaly after chromosomal examination,7 pregnant women were found with trisomy 18,and one pregnant woman was found with 47,XX,+M[4].
