伴自身免疫性疾病的胸腺脂肪瘤3例临床病理观察

摘　　要：目的探讨伴自身免疫性疾病的胸腺脂肪瘤临床病理特征、诊断与鉴别诊断。方法应用光镜和免疫组化对3例伴自身免疫性疾病的少见胸腺脂肪瘤进行临床病理分析,并复习相关文献。结果 2例胸腺脂肪瘤伴重症肌无力,1例伴甲亢。镜下见肿瘤由成熟的脂肪组织和胸腺组织以不同比例混合构成,常见胸腺小体结构。结论胸腺脂肪瘤是罕见的纵隔良性肿瘤,可伴自身免疫性疾病,其独特的组织病理学形态是诊断和鉴别诊断的主要依据。Objective To investigate the clinicopathological features,diagnosis and differential diagnosis of the thymolipomas with autoimmune diseases.Methods The clinicopathological features and immunohistochemical examination were analyzed in three cases of thymolipoma with autoimmune diseases and the related literatures were reviewed.Results Two cases of thymolipoma associated with myasthenia gravis,and one combined with hyperthyroidism.Under microscope the thymolipoma was consisted of mature fatty tissue and thymic tissue structures with Hassall＇s corpuscles.Conclusion Thymolipoma is a rare benign tumour composed of thymic elements and adipose tissue.It may associate with autoimmune diseases.The diagnosis and differential diagnosis entirely base on the specific histopathological characteristics.
