膨胀期白内障继发青光眼手术治疗的临床观察

摘　　要：目的探讨不同手术方式治疗膨胀期白内障继发青光眼的疗效。方法 53例(56眼)膨胀期白内障继发青光眼患者根据病情分为A组30眼和B组26眼,A组行白内障超声乳化吸除加人工晶状体植入术,B组行超声乳化白内障吸除联合人工晶状体植入加小梁切除术。结果 2种手术方式术后矫正视力、眼压、房角开放情况大体相当,2组合计矫正视力≥0.3者39眼(69.6%),42眼(75%)眼压恢复正常,39眼(69.6%)房角完全开放。结论膨胀期白内障继发青光眼患者根据房角开放程度选择合适的手术方式,能有效控制眼压,提高视力,避免单眼的多次手术。Objective To investigate the efficacy of different surgical treatment for secondary glaucoma of expansion cataract. Methods According to the situation of disease, 53 cases （56 eyes） of expansion cataract and secondary glaucoma patients were treated with phacoemu!sification cataract extraction plus intraocular lens implantation, or phacoemulsification cataract extraction and intraocular lens implantation plus trabeculectomy.Results The visual acuity, intraocular pressure, and the angle opening in two surgical groups were roughly same, both groups combined corrected visual acuity≥ 0.3 were in 37 eyes （66.1%）, the intraocular pressure of 42 eyes （12.5%） returned to normal; and 39 eyes angle （69.6 % ） were completely opened. Conclusion According to the angle opening degree of secondary glaucoma of expansion cataract, by selecting the appropriate surgical approach, intraocular pressure can be effectively controlled and vision can be improved, and which can avoid unilateral surgery.
