翼状胬肉切除联合羊膜移植手术前后泪膜功能的改变

摘　　要：目的探讨翼状胬肉患者行胬肉切除联合羊膜移植手术前后泪膜功能的变化。方法对44例(44眼)翼状胬肉患者行胬肉切除联合羊膜移植术,于术前3天、术后1周、4周、12周分别进行泪液分泌(SchirmerⅠ)试验、泪膜破裂时间(breakuptime,BUT),印迹细胞学检查。结果术前3天和术后各时间段泪液分泌量比较差异无统计学意义(P>0.05)。术后1周较术前3天泪膜破裂时间明显缩短(P<0.01),术后4周、术后12周较术前3天泪膜破裂时间延长(P<0.05)。印迹细胞学检查表明术后1周检查结果较术前3天杯状细胞减少(P>0.05),术后4周及术后12周检查结果示杯状细胞的密度明显增加(P<0.05)。结论翼状胬肉切除联合羊膜移植术可使患者泪膜功能得到一定程度的改善。【Objective】 To investigate the changes of tear film function in pterygium patients row pterygium excision with amniotic membrane transplantation before and after operation.【Methods】 A total of 44 cases（44 eyes） underwent pterygium excision with amniotic membrane transplantation.Schirmer test,tear break-up time,impression cytology were measured 3 day before the surgery and repeated 1 week,4 weeks and 12 weeks after the surgery.【Results】 There was no significant difference statistically in the amount of tear secretion between 3 day before the surgery and the different time slots after the surgery（P 0.05）.Compared with before operation,BUT were decreased in patients 1 weeks after operation（P 0.01）.Compared with before operation,BUT were prolonged in patients 4 weeks and 12 weeks after operation（P 0.05）.In impression cytology,goblet cells density were decreased in patients 1 weeks after operation（P 0.05） while increased in patients 4 weeks and 12 weeks after operation（P 0.05）.【Conclusions】 Pterygium excision with amniotic membrane transplantation can improve the tear film function in pterygium patients.
