胰腺实质性假乳头状肿瘤的影像学诊断

摘　　要：目的:探讨胰腺实质性假乳头状肿瘤(SPTP)的影像学特点。方法:回顾性分析2000～2010年经手术及病理证实的6例SPTP患者影像学资料,并与病理结果相对照。结果:病灶体积较大,多位于胰腺体、尾部,形态不规则,边界多清晰。超声下多表现为包膜完整的囊实性混杂回声肿块,可见钙化灶,分布不均匀,彩色多普勒可见肿块内有少许血流信号。CT可清楚显示肿块呈囊实性混杂密度,显示肿块内钙化优越,增强后肿块实性部分呈渐进性中等强化。MRI肿块常表现为混杂信号影。结论:胰腺实质性假乳头状肿瘤具有较为典型的影像学表现,综合影像学表现并结合临床有助于本病的诊断。Objective：To analyze the imaging characteristics of solid pseudopapillary tumors of pancreas（SPTP）.Methods： The imaging data collected between 2000 and 2010 in 6 patients with SPTP confirmed surgically and pathologically were retrospectively analyzed and compared with the pathological findings.Results： The lesion was generally large-sized and located either at the body of pancreas or cauda pancreatis,with irregular shape but clear boundary definition.Ultrasonographic examination revealed well-encapsulated cystic and solid mass with hypoechoic/isoechoic echotexture and unevenly distributed calcifications.Color Doppler flow imaging demonstrated slight blood flow signal in the mass.Plain CT of the abdomen clearly revealed mixed internal cystic mass ratio,and solid part was seen on contrast-enhanced image.MRI images often showed mixed signal due to diversity components of the tumor.Conclusion： SPTP has certain typical imaging properties,thus diagnostic imaging with clinical examination is helpful for the final diagnosis of this disease.
