凉山地区彝族2型糖尿病患者焦虑和抑郁状态及影响因素分析

摘　　要：目的调查凉山地区彝族2型糖尿病患者的焦虑扣抑郁状态及影响因素。方法选择凉山一院入院治疗的168例彝族2型糖尿病患者及186例汉族2型糖尿病患者进行病例对照研究，并采用自设的基本情况问卷、抑郁自评量表（SDS）、焦虑自评量表（SAS）测量研究对象的心理健康状况，通过t检验、Logistic回归分析等对相关因素进行分析。结果彝族患者组43．45％合并抑郁状态，34．35％的患者合并焦虑状态，均高于汉族患者组（38．93％与29．26％）（P〈0．05，P〈0．05），而且彝族患者组合并中重度抑郁及焦虑状态患者的比例明显高于汉族患者组。结论加强凉山彝族地区彝族糖尿病患者的糖尿病健康教育及心理健康状况的干预是必要的，尤其是年龄大，并发症多，文化水平低及经济水平差的彝族糖尿病患者。Objective To investigate the psychological state of type 2 diabetic patients in Liangs- han Yi Autonomous Prefecture and analyze the influential factors on anxiety and depression. Methods 168 patients with type 2 diabetes of Yi--people and 186 patients with type 2 diabetes of Han--people were collected by a ease--control study in First Hospital of Liangshan. Subjects were assessed with con- sulting questionnaires of potential risk factors, Self-- Rating Depression Scale （SDS） and Self-- Rating Anxiety Scale（SAS）. T--test,logistic regression were used to analyze the psychological factors in the subjects. Results The incidences of depression and anxiety were 43.45% and 34.35% respectively in patients with type 2 diabetes of Yi--people, which were higher than that in the patiens with type 2 diabe- tes of Han--people （38.93% and 29.26%）（P〈0.05）. Furthermore, the incidences of the moderate to severe depression and anxiety in diabetic patiens of Yi--people were much higher than in diabetic patiens of Han--people. Conclusions It is necessary to strengthen the diabetes health education and mental health intervention with diabetic patiens of Yi-- people, especially the elder, with more complications, low level of education and the low income patients.
