摘 要:目的探讨彩色多普勒超声探测子宫颈癌组织的血流信号和免疫组化法检测的肿瘤内微血管密度(MVD)的相互关联及其与宫颈癌临床病理的关系。方法收集临床资料,采用彩色多普勒超声探测38例子宫颈癌和45例正常宫颈组织内血流信号,计测阻力指数(RI);采用免疫组化技术检测肿瘤MVD。结果子宫颈癌患者的MVD显著高于正常宫颈组织(P<0.01);子宫颈癌组织内血流RI值显著低于正常宫颈组织(P<0.01)。宫颈癌肿瘤组织内血流RI值和MVD与组织学分级、临床分期有关(P均<0.05),而与淋巴结转移、组织学分型无关(P均>0.05)。子宫颈癌肿瘤组织内血流RI值与免疫组化检测的肿瘤MVD呈负相关(r=-0.489,P=0.018)。结论彩色多普勒超声检测子宫颈癌血流RI值与免疫组化检测肿瘤MVD从不同角度反映子宫颈癌的血管生成情况,有利于对子宫颈癌血管生成及预后的评价。Objective To evaluate the relationship between intratumoral blood flow signal derived from color Doppler and microvessel density (MVD)derived from immunohistochemical method and their relevance in cervical carcinoma and the correlation with the clinico-pathological features of the cancer.Methods Before operation, the intratumoral flow signal of cervical cancer and resistance index (RI) of the lesions were detected by color Doppler ultrasonography in 38 cases of cervical carcinoma and 45 cases of normal cervix. MVD of excised tumor was assessed by immunohistochemical S-P method.Results MVD in tissues of cervical carcinoma was higher than that in normal cervical tissues(P0.01). RI in lesions of cervical carcinoma was lower than that in normal cervical tissues(P0.01). Either RI or MVD in cervical carcinoma was correlated with histologic grading and clinical staging(all P0.05); however, neither RI nor MVD in cervical carcinoma was correlated with histologic type and lymph-node metastasis (all P0.05). Univariate linear analysis revealed that there was a negative correlation between MVD and RI (r=-0.489 , P=0.018).Conclusions RI and MVD could reflect the angiogenesis of cervical carcinoma from different point of view and are beneficial to evaluating the angiogenesis and prognosis of cervical carcinoma.
