摘　　要：目的研究尿细胞角蛋白CYFRA 2l-1在膀胱癌中的表达。方法选择我院2011年12月至2012年5月32例经病理确诊的膀胱癌作为观察组,32例膀胱良性病变患者作为良性组,同时选择同期健康体检者32例为对照组,均取晨尿,对3组CYFRA 21-l进行检测,对CYFRA 21-l表达进行研究,同时对观察组患者不同病理分期、治疗前后CYFRA2l-1变化情况进行观察。结果观察组为(29.52±18.33)ng/ml,良性组为(4.47±2.33)ng/ml,观察组CYFRA21-1大大高于良性组与对照组,对照组CY-FRA21-1高于正常组,组间比较差异具有显著性,有统计学意义(P<0.05),浅表性膀胱癌CYFRA21-1检测值为(8.49±2.21)ng/ml,大大低于浸润性膀胱癌,两组比较差异具有显著性,有统计学意义(P<0.05),经手术、膀胱灌洗、放疗、化疗等治疗后,浅表性膀胱癌组CYFRA21-1下降明显,接近临界值,与治疗前比较差异具有显著性,有统计学意义(P<0.05),浸润性膀胱癌患者治疗后较治疗前略有下降,但下降幅度不大。结论尿CYFRA 2l-l检测对膀胱癌的诊断及预后判定均有较高的临床价值。Objective To study the urine cytokeratin CYFRA 2 1-1 the expression in bladder cancer. Methods Choose from December 2011 to 2012 May 32 cases diagnosed by pathology of bladder cancer as ob- servation group, 32 cases of patients with bladder benign lesions as benign group, at the same time, choose the same period the healthy check-up 32 cases as control group, all take morning urine, of three groups of CYFRA 21-1 testing of CYFRA 21-1 expression, at the same time to observe groups were different pathological stages be- fore and after treatment, CYFRA 2 1-1 changes were observed. Results Observation group （29. 52 ± 18. 33） μ g/L, benign group （4. 47 ±2. 33）μ g/L, the observation group CYFRA21-1 greatly higher than benign group and control group, control group CYFRA21-1 was higher than normal group, was compared between group with significant difference, there was statistical significance （P 〈 0.05 ）, superficial bladder cancer CYFRA21-1 val- ues for （8.49 ± 2. 21 ） μg/L, greatly lower than invasive bladder cancer, two groups of comparisons with signifi- cant difference, there was statistical significance （ P 〈 0.05 ）, the surgery, bladder douche, radiotherapy, chemotherapy and after treatment, superficial bladder cancer group CYFRA21-1 down significantly, close to the critical value, and before treatment compared with significant difference, there was statistical significance （P 〈 0. 05 ）, invasive bladder cancer patients after treatment than before treatment will drop slightly, but the decline range is not too much. Conclusion Urinary CYFRA 2 M detection to the diagnosis of bladder cancer and prog- nosis judge all have high clinical value.
