血液滤过并静脉壶抽脂治疗高脂血症性重症急性胰腺炎临床分析

摘　　要：目的评估血液滤过并静脉壶抽脂治疗高脂血症性重症急性胰腺炎(HL-SAP)的疗效。方法 24例高脂血症性重症急性胰腺炎的患者,分为血液滤过并静脉壶抽脂治疗组14例和对照组10例,观察两组滤器使用寿命、总血液滤过时间、血液滤过前及血液滤过后72h的血清甘油三酯(triglycer-ide,TG)、急性生理和慢性健康状况评分Ⅱ(APACHEⅡ评分)、病死率等。结果治疗组滤器使用寿命、总血液滤过时间、血液滤过后72h时TG与对照组比较差异有统计学意义(P<0.05);血液滤过后7 2h APACHEⅡ评分、病死率两组比较无统计学意义(P>0.05)。结论血液滤过并静脉壶抽脂治疗高脂血症性重症急性胰腺炎能延长滤器使用寿命,加快TG下降速度,缩短血液滤过时间。Objective To evaluate the effect of hemofiltration combined with liposuctionon by venous chamber on the treatment of patients with hypertriglyceridemia severe acute pancreatitis. Methods 24 patients with hypertriglyceridemia severe acute pancreatitis were sutdied. 14 patients were treated on hemofiltration com- bined with liposuctionon by Venous chamber,10 patients were treated on hemofiltration. Service life of filters, total hemofiltration time, before and after hemofiltration for 72 hours, serum concentration of triglyceride, A- PACHE lI score, and mortality between the two groups were observed. Results There was statistical signifi- cance（P 〈0. 05） in service life of filters, total hemofiltration time and serum concentration of triglyceride after hemofiltration for 72 hours between the two groups. There was no statistical significance （P 〉 0. 05 ） in A- PACHE Ⅱ score, and mortality after hemofiltration for 72 hours between the two groups. Conclusion Hemofil- tration combined with liposuctionon by venous chamber is superior to hemofiltration in the treatment of hypertrig- lyceridemia severe acute panereatitis, and can prolong service life of filters, accelerate reducing the concentra- tion of triglyceride, shorten hemofiltration time.
