脑内原发性淋巴瘤的磁共振诊断

摘　　要：目的探讨脑内原发性淋巴瘤的磁共振表现及其诊断价值。方法回顾性分析12例经手术病理证实的脑内原发性淋巴瘤MRI影像资料,记录病变位置、数目、信号特点及强化特点等。结果 12例脑内淋巴瘤单发9例,多发3例。共18个病灶,其中幕上16个,幕下2个。磁共振平扫T1WI呈等或稍低信号,T2WI呈等或稍高信号,15个病灶信号均匀,3个信号不均匀。增强扫描均见瘤体均匀或不均匀强化;瘤周多为轻、中度水肿。结论脑内原发性淋巴瘤磁共振表现具有一定的特征性,在术前有可能正确诊断。Objective To discuss the MRI features and diagnostic value of primary intracranial lymphoma.Methods Twelve patients with primary brain lymphoma proved by histopathology were collected.All of them were performed with MR imaging,and the location,number,signal intensity and enhancement feature of tumors were retrospectively reviewed.Results 9 patients with single lesion were found and 3 patients with multiple lesions,There were 18 lesions from 12 patients at all,16 lesions were located at supratentorial region and other 2 lesions located at infratentorial region.In MRI imaging,The lesions demonstrated isointensity signal or slightly hypointensity signal on T1WI,and isointensity signal or slightly hyperintensity signal on T2WI,After Gd-DTPA injection,Homogeneous and asymmetry enhancement were showed in lesions.Slight and moderate edema was found in para-tumorous region.Conclusion MRI findings of primary brain lymphoma have certain characteristic features,with which can diagnose correctly before surgeon.
