摘  要:目的:探讨急性肝功能衰竭大鼠血清与组织中降钙素原(PCT)水平的变化,及其与细胞因子之间的关系。方法:将25只雄性大鼠随机分为正常组、模型组。用D-氨基半乳糖盐酸盐诱导大鼠急性肝功能衰竭模型。采用ELISA法检测大鼠血清及肝脏、脾脏、小肠组织中的PCT水平,以及细胞因子TNF(肿瘤坏死因子)-α、IFN(干扰素)-γ、IL(白介素)-4血清水平。采用鲎试剂检测内毒素水平。采用HE染色观察肝组织病理形态学改变。结果:正常组大鼠血清PCT、TNF-α、IFN-γ、IL-4及内毒素水平很低,而模型组大鼠12小时、24小时血清PCT、TNF-α、IFN-γ、IL-4及内毒素水平均显著高于正常组(P<0.05)。正常组大鼠的肝脏、脾脏及小肠组织中均可检测到PCT,但水平较低;而模型组大鼠12小时、24小时的肝脏、脾脏及小肠组织中PCT水平显著高于正常组(P<0.05)。结论:血清PCT水平的升高是诊断急性肝衰竭早期并发感染的特异性及敏感性指标。肝脏、脾脏及小肠组织中PCT水平变化比血清PCT更敏感,可能与器官功能的变化有一定关系。Objective:To investigate the changes of procalcitonin(PCT) levels in serum and tissue of the rats with acute liver failure,and the relationship between PCT and cytokines.Methods:Twenty-five male rats were randomly divided into normal group and model group.The rats in model group were induced acute liver failure using D-galactosamine.The PCT levels in serum and tissue,including liver,spleen and small intestine,and the cytokine levels,including TNF-α,IFN-γ and IL-4,in serum were all detected by ELISA.The endotoxin levels in serum were assayed using LAL test.The histological changes of liver tissue were observed by HE stain.Results: The levels of serum PCT,TNF-α,IFN-γ,IL-4 and endotoxin were very low in normal group,but the levels of serum PCT,TNF-α,IFN-γ,IL-4 and endotoxin were all significant higher in model group at 12h or 24h than in normal group(P〈0.05).The low levels of PCT could be detected in liver,spleen,small intestine in the rats of normal group,but the levels of PCT in above tissues were all significant higher in model group at 12h or 24h than in normal group(P〈0.05).Conclusion:A high level of PCT is a sensitive and specific parameter in the diagnosis of acute liver failure complicated early with infection.The changes of PCT levels in liver,spleen and small intestine were more sensitive than in serum,which may be related with the changes of organic function.
