摘 要:目的:探讨甲钴铵治疗对降低脑卒中病人血浆同型半胱氨酸的效果。方法:选择脑卒中住院患者360例,分为脑梗死组260例,脑出血组100例,另选同期住院非脑卒中患者200例(对照组),测定其Hcy、GLU、TG、CHOL、HDL、LDL-C、FIB的水平。随机选取360例脑卒中伴高Hcy血症患者中133例(脑梗塞95例,脑出血38例),随机分为常规治疗组、甲钴胺组,半个月后再次抽血检测血浆Hcy。结果:脑梗死组和脑出血组的Hcy含量分别为19.08±10.33umol/L、18.76±10.23umol/L,与对照组(12.17±4.61umol/L)相比显著升高,具有统计学意义(P<0.01)。甲钴胺组Hcy浓度治疗后明显下降,常规治疗组治疗后Hcy浓度无明显下降,两组治疗在降低血Hcy浓度方面比较差异有统计学意义(P<0.01)。结论:高同型半胱氨酸血症是脑卒中的独立危险因素,甲钴铵治疗能降低脑卒中患者的平均Hcy水平。Objective: To explore the treatment effect of vancomin on lowering homocysteine of patients with cerebral stroke.Methods: 360 cases of patients with cerebral stroke were chosen and divided into the cerebral infarction group with 260 cases and the cerebral hemorrhage group with 100 cases;200 contemporary non-stroke patients in the neurologic department were chosen as the control group.The levels of Hcy,GLU,TG,CHOL,HDL,LDL-C and FBI of the three groups were detected.Among the 360 cases,133 cases of cerebral stroke accompanied by hyperhomocysteinemia,including 95 cases of the cerebral infarction and 38 cases of the cerebral hemorrhage,were randomly chosen and divided into the conventional therapy group and the mecobalamin group,in which the Hcy of blood plasma was detected half a month later.Results: Hcy contents of the cerebral infarction group and the cerebral hemorrhage group were respectively 19.08±10.33umol/L,18.76±10.23umol/L,obviously higher than 12.17±4.61umol/L of the control group,which was of statistical significance(P0.01).Concentration of Hcy in the mecobalamin group declined obviously after treatment,while no conspicuous decline appeared in the Hcy concentration of the conventional therapy group;in terms of lowering the Hcy concentration,the difference between the two groups was of statistical significance(P0.01).Conclusion: Hyperhomocysteinemia is an independent risk factor in the cerebral stroke and the mecobalamin therapy can reduce the average Hcy level of patients with cerebral stroke.
