彩超引导下两种真空辅助旋切系统在乳腺微创外科中的应用

摘　　要：目的:探讨彩超引导下两种真空辅助乳腺旋切系统在乳腺微创外科中的价值。方法:彩超引导下对480例1 194个乳腺病灶进行微创切除,同期对210例374个乳腺病灶进行开放切除,比较术中、术后并发症和手术时间、疤痕长度等。结果:微创组与开放组在出血、皮下血肿、肿瘤残留、皮肤破损、感染方面有统计学差异(P<0.05),术后复发方面无统计学差异(P>0.05);微创组手术时间短,疤痕小,同开放组比较具有统计学差异(P<0.05)。麦默通组与安珂组在术中出血、局部瘀斑、局部血肿及皮肤破损、感染、复发等术中、术后并发症方面无统计学差异(P>0.05),肿瘤残留方面安珂比麦默通稍高,但无统计学差异(P>0.05)。结论:超声引导下采用麦默通或安珂微创旋切系统,优势明显、操作简单、并发症少、美观安全,值得推广。Objective. To investigate application value of two vacuum assisted biopsy system under Color ultrasound guidance in breast minimally invasive surgery. Methods. A total of 480 cases with 1 194 breast lesions underwent color ultrasound guided minimally invasive surgery, another 210 cases with 374 breast lesions underwent opening resection. Complications during and after surgery, surgery duration, scar length etc were observed and compared. Results. The differences were significant in bleeding, subcuta- neous hematoma, residual tumor, skin damage and infection （P〈0.05）, but it was not significant in post- operative recurrence （P〉0. 05）; minimally invasive group had significantly shorter duration and smaller scars （P〈0.05）. And there was no significant difference in blood loss, local bruising, local hematoma, and complication during surgery or after surgery including skin damage, infection, recurrence between Mammotome group and EnCor group （P〉0.05）. The incidence of residual tumor was higher in EnCorgroup, but the difference was not significant （P〉O. 05）. Conclusions= Using Mammotome or EnCor mini- mally invasive biopsy system under color ultrasound guidance has obvious advantages. It has advantages of simple procedure, little complication, beautiful outlook and good safety,it＇s worthy of elinieal promotion.
