摘 要:目的了解体表刺激法记录复合肌动作电位(CMAP)对大鼠坐骨神经损伤后电生理恢复的动态监测作用。方法建立SD大鼠坐骨神经钳夹伤模型,术后2、4、6周通过足迹试验测定坐骨神经功能指数(SFI),并通过体表刺激记录术侧腓肠肌CMAP,测量CMAP平均振幅及运动神经传导速度(MCV)。结果术后各时间点缺损组大鼠均未记录到CMAP,SFI约为-100;而假手术组大鼠均能记录到稳定的CMAP,SFI值0左右。夹伤组大鼠术后SFI逐渐恢复,术后6周时与假手术组相比差异无统计学意义。坐骨神经夹伤后2周未记录到CMAP,4周时CMAP波幅较小,6周时CMAP振幅大约恢复到假手术组的80%,MCV大约是假手术组的58%。结论通过体表刺激可稳定地记录神经再生过程中的复合肌动作电位,因此体表刺激记录法可无创、动态地监测神经再生过程中的神经传导电生理恢复。Objective To evaluate the dynamic observation of electrophysiological recovery of rats with sciatic nerve crush injury by using the surface stimulation method to record compound muscle action potential(CMAP).Methods Thirty adult female SD rats were randomly divided into three groups – rats subjected to sciatic nerve crush injury,defect injury and sham-operated group.Sciatic function index(SFI) was measured by walking track analysis in the second,fourth and sixth week after operation respectively.The CAMP of gastrocnemius on the operated side,the average amplitude of CAMP and the motor nerve conduction velocity(MCV) were also measured during the same period.Results CAMP of the defect injury group was not observed at any time after operation,with SFI value about-100,while stable CAMP was observed in the sham group,with SFI value around 0.SFI was gradually restored in the crush group,which was not statistically different compared with the sham group in the sixth week after operation.The CAMP was not observed in the second week after crush injury but low amplitude CAMP could be detected in the fourth week.In the sixth week,the CAMP was about 80% of the sham group and the MCV was about 58% of the sham group.Conclusion CAMP during recovery can be well-recorded by surface stimulation so that this method can be used to dynamically observe electrophysiological recovery during the regeneration of nerve without trauma.
