CT引导下^(125)I粒子植入联合胰腺动脉灌注治疗中晚期胰腺癌

摘　　要：目的探讨CT引导下125I粒子植入联合胰腺动脉灌注(PAI)治疗中晚期胰腺癌(APC)的临床应用价值。方法将49例APC患者分成125I粒子植入联合PAI组(A组,n=21)和单纯PAI组(B组,n=28)。A组中16例伴肝转移,其中14例同时接受TACE,2例接受经皮穿刺肝肿瘤无水乙醇注射(PEI);B组中20例伴肝转移,其中17例同时接受TACE,3例接受PEI。对比两组治疗后的整体有效率、临床受益率、疼痛缓解程度、生存期及术后并发症。结果 A组整体有效率、临床受益率分别为61.90%(13/21)和76.19%(16/21),均高于B组的32.14%(9/28)、46.43%(13/28),差异均有统计学意义(P均<0.05);A组中疼痛评分降低>50%者达到76.47%(13/17),高于B组的26.09%(6/23,P<0.05);A组中位生存期为286天,明显高于B组的194天(P<0.05)。结论 125I粒子植入联合PAI对APC疗效确切,能改善患者生存质量,缓解疼痛,提高生存率。Objective To evaluate the clinical efficacy of lzsi seed implantation in combination with pancreatic arterial in： fusion in treating advanced pancreatic cancer （APC）. Methods Forty-nine patients suffered from APC were randomly di- vided into two groups. Patients in group A （n~ 21） underwent 12sI seed implantation plus pancreatic arterial infusion （PAD, in group B （n^28） underwent single PAL Sixteen patients in group A had liver metastases, 14 of which under- went TACE, and 2 were treated with percutaneous ethanol injection （PEI）. In group B, 20 patients had concurrent liver metastases, 17 of which underwent TACE, and 3 were treated with PEI. The curative effect of the two groups were evalu- ated through response rate （RR）, clinical benefit rate （CBR）, degree of pain relief, survival and adverse reactions. Results RR and CBR of group A was 61.90% （13/21） and 76.19% （16/21）, respectively, while of group B was 32.14% （9/ 28） and 46, 43% （13/28）, respectively （all P〈0.05）. In group A, more than 50% pain score decrease was observed in 76.47% patients, higher than that in group B （26.09%, P^0. 05）. In addition, the median survival time of the group A （286 days） was longer than that of group B （194 days, P〈0.05）. Conclusion^ 125I seed implantation plus PAI has obvious therapeutic effect of APC, and can improve the quality of life, relieve pain, and improve survival rate.
