视杯深浅与NAION、CRVO、BRVO及DR发病的相关性

摘　　要：目的采用光学相干断层扫描仪检测非动脉炎性前部缺血性视神经病变(NAION)、视网膜中央静脉阻塞(CRVO)、视网膜分支静脉阻塞(BRVO)、糖尿病性视网膜病变(DR)患者及正常人的视杯深度,探讨视杯深浅程度与发病的相关性。方法纳入NAION、CRVO、BRVO及DR患者各80例80只眼,NAION、CRVO、BRVO者选择对侧眼,DR者随机选取1只眼,应用德国Heidelberg谱域光学相干断层扫描仪检查系统(SD-OCT)进行视盘"十字交叉"扫描,观察其视杯深度,并以80例随机选择单侧80只正常眼视杯扫描结果作为对照,对各眼的视杯深浅程度进行统计分析。结果依据视杯深浅不同程度分为3级。NAION组对侧眼中I级视杯29只眼,Ⅱ级视杯44只眼,Ⅲ级视杯7只眼;CRVO组对侧眼中I级视杯8只眼,Ⅱ级视杯58只眼,Ⅲ级视杯14只眼;BRVO组对侧眼中I级视杯7只眼,Ⅱ级视杯36只眼,Ⅲ级视杯37只眼;DR组随机选择观察眼中I级视杯6只眼,Ⅱ级视杯24只眼,Ⅲ级视杯50只眼;正常对照眼组中I级视杯6只眼,Ⅱ级视杯26只眼,Ⅲ级视杯48只眼。统计学分析结果显示,NAlON组、CRVO组与正常组比较存在差异(P<0.01),均浅于正常人,NAlON组视杯较CRVO组更为窄浅(P<0.05)。DR组、BRVO组与正常组比较无统计学意义的差异(P>0.05)。结论 BRVO、DR不同视杯分级与正常对照人群相似,而NAlON、CRVO者I级、Ⅱ级视杯者明显高于正常对照者,提示NAION、CRVO者的发病与视盘解剖中视杯杯底位置异常有关。OBJECTIVE To explore the correlation between the depth of optic cup and pathogenesis with nonarteritic anterior ischemic optic neuropathy（NAION）,central retinal vein occlusion（CRVO）,branch retinal vein occlusion（BRVO）,and diabetic retinopathy（DR） using optical coherence tomography.METHODS We detected optic cup depth of patients with NAION,CRVO,BRVO,DR and normal human.Each group included 80 eyes.German Heidelberg spectral domain optical coherence tomography（SD-OCT） examination system for optic disc ＂cross＂ scanning was used.Three subgroups were graded in different optic cup depth.NAION and CRVO groups were selected contralateral eyes,and BRVO group was chosen affected eyes,at the same time,in DR patients and control group only one eye accepted detection according to random selection.The degree of optic cup depth and incidence were observed and analyzed..RESULTS Grade I optic cup of NAION group contralateral eyes was in 29 eyes,grade Ⅱ optic cup in 44 eyes,grade Ⅲ optic cup in 7 eyes;grade I optic cup of group CRVO contralateral eyes was in 8 eyes,grade Ⅱ optic cup in 58 eyes,grade Ⅲ optic cup in 14 eyes;grade I optic cup of group BRVO affected eyes was in 7 eyes,grade Ⅱ optic cup in 36 eyes,37 eyes of grade Ⅲ optic cup;grade I optic cup of group DR randomly chosen eyes was in 6 eyes,grade Ⅱ optic cup in 24 eyes,grade Ⅲ optic cup in 50 eyes;and grade I optic cup of normal control eyes was in 6 eyes,grade Ⅲ optic cup in 26 eyes,grade Ⅲ optic cup in 48 eyes.Statistical analysis of the results showed that compared with the normal group,NAlON group and CRVO group were significantly different（P0.01）.Compared NAlON group with CRVO group the cup was narrower and shallower,and the difference was significant（P0.05）.DR group,BRVO group compared with the normal group in optic cup subgroup showed no significant difference（P0.05）.CONCLUSIONS The grading of subgroup optic cup of BRVO,DR and normal control group was similar,but NAlON group,CRVO group,no optic cup observed i
