联合检测CA125、HE4和RBP4对预测上皮性卵巢癌的临床价值评价

摘　　要：目的探讨联合检测肿瘤相关抗原125(CA125)、人附睾蛋白4(HE4)和人视黄醇结合蛋白4(RBP4)在预测早期上皮性卵巢癌及分期的临床价值。方法选择80例上皮性卵巢癌患者作为卵巢癌组,80例健康妇女作为正常对照组,分别测定其血浆CA125、HE4和RBP4水平。按照不同的组合方式比较各组合预测上皮性卵巢癌及分期的敏感性和特异性。结果单个肿瘤标志物检测以RBP4的敏感性最高(90%),CA125的特异性最高(95%)。联合两个肿瘤标志物检测以CA125+RBP4组合的敏感性最高(95%),而CA125+HE4组合的特异性最高(87.5%)。在预测卵巢癌分期时,单个肿瘤标志物检测以RBP4的敏感性最高(早期81.2%,晚期95.8%),而联合两个肿瘤标志物检测早期卵巢癌以CA125+RBP4组合的敏感性最高(93.8%)。结论应用CA125联合RBP4检测有助于卵巢癌的筛查且有助于预测卵巢癌的分期。Objective To investigate the value of different combinations of CA125, HE4 and RBP4 in predicting the epithelial ovarian cancer and its stage. Methods A prospective case-control study design including 80 women with epithelial ovarian cancer and 80 healthy women. The serum concentration of CA125, HE4, and RBP4 were measured. The diagnostic performance of various decision-rules for combinations of these tests was assessed to evaluate potential use in predicting epithelial ovarian cancer. Results The RBP4 has the highest sensitivity to predict the epithelial ovarian, but the CA125 has the highest specificity. In two biomarkers combination, the CA125＋RBP4 have the highest sensitivity, and the CA125＋HE4 has the highest specificity. In predicting the stage of epithelial ovarian cancer, the CA125＋RBP4 has the highest sensitivity. Conclusions The combination of CA125 and RBP4 yields acceptable specificity, and higher sensitivity than other combinations. It may be useful in predicting the epithelial ovarian cancer and its stage.
