腹部肿瘤放疗真空袋体位固定的应用

摘　　要：目的探讨腹部肿瘤病人放射治疗使用真空袋体位固定技术的临床应用价值。方法 52例腹部肿瘤病人体外照射随机分为真空袋组26例和非真空袋组26例,所有病人分别在首次和每周周末治疗时,在直线加速器下摄取验证片,通过与定位片比较测出误差大小。结果两组摆位误差结果显示:前后方向和左右方向误差均有统计学意义(t=11.88,p<0.01;t=8.64,p<0.01),上下方向两组差异无统计学意义(t=1.86,p>0.05),说明真空袋组在前后和左右方向摆位误差明显少于非真空袋组,在上下方向无明显减少误差。结论腹部肿瘤病人放射治疗中,使用真空袋体位固定可减少前后和左右两个方向摆位误差,并使摆位简单易行,提高摆位精确度和疗效,病人体位舒适,具有临床推广价值。Objective To investigate the clinical value of abdominal cancer patients radiation therapy using the vacuum bag immobilization technology. Methods 52 abdominal cancer patients were randomly divided into vacuum bag group of 26 cases and non - vacuum bag group of 26 cases. The film was taken at the first and each weekend with the treatment position in simulator for each patient, then comparing these films with primary simulation film, and set - up uncertainty was measured. Results Set - up uncertainty of two groups the results show ： there was signifi- cant difference in antero - posterior（ A - P） and medio - lateral（ M - L） directions of the two groups（ t = 11.88, p 〈 0. 01 ; t = 8.64 ,p 〈 0. 01 ） , there was no significant difference in Up - under （ U - U ） direction of the two groups（ t = 1.86 ,p 〉 0. 05 ）, which explained in A - P and M - L directions of the set - up uncertaintys was significantly lower in vacuum bag than that in non - vacuum bag group , the U - U direction of vacuum bag group did not reduce the set - up uncertainty. Conclusion Using vacuum bag can reduce in A - P and M - L directions of the set - up uncertaintys in external radiotherapy of abdominal cancer patients, the ste - up procedure is simple and easy, improve the set - up precision and treatment effect of radiotherapy, patient posture comfort, which has the value of clinical practice.
