心理疏导联合抗抑郁及焦虑治疗伴焦虑及抑郁症状的NERD的疗效

摘　　要：目的探讨非糜烂性胃食管反流病(non-erosive reflux disease,NERD)抑郁、焦虑症状的患病情况及对心理疏导联合药物疗效的疗效。方法选择NERD366例应用综合医院焦虑、抑郁量表(the hospital anxiety and depressive scale,HADS)进行心理测评,对并发抑郁和(或)焦虑症状患者,按单纯抑郁症状、单纯焦虑症状及抑郁合并焦虑症状随机,抽取120例分成治疗A、B、C组和对照A、B、C组,每组20例。各对照组予雷贝拉唑10mg,1次/天,口服,枸橼酸莫沙比利5mg,3次/天,口服,黛力新1片、2次/天,口服;各治疗组在对照组用药基础上给予心理疏导,疗程为8周,并于治疗后2、4、6、8周观察反酸、胃灼热、恶心、胸骨后疼痛及抑郁和(或)焦虑症状的缓解情况。结果 NERD患者的抑郁和(或)焦虑患病率为82.5%;各治疗组治疗8周后,精神和躯体症状均改善,与对应对照组比较差异有非常显著的统计学意义(P<0.01)。结论 NERD患者有较高的抑郁和(或)焦虑患病率,在常规治疗基础上加用抗抑郁药的同时给予心理疏导能显著改善精神和躯体两方面的症状。Objective To investigate the role of depressive symptoms,anxiety symptoms in non-erosive reflux disease（NERD）and effect of Psychological counseling jointing antidepressant drug.Methods The Hospital Anxiety and Depressive Scale（HADS）was applied to assess the psychological analysis of 366 cases of NERD.120 NERD patients who had depressive symptoms and（or）anxiety symptoms were randomly divided into treatment groups A,B and C,and control groups A,B,and C.Each group has 20 cases.Treatment groups who were given Psychological counseling for 8 weeks and Rabeprazole 10 mg once a day,Citrate Mosapride 5 mg third a day,Deanxit twice a day.Control groups who were given Rabeprazole 10 mg Once a day,Citrate Mosapride 5 mg third a day,Deanxit twice a day.Major clinical symptoms,depressive symptoms and（or）anxiety symptoms were evaluated after 2,4,6,8 weeks.Results The rate of patients who had depressive symptoms and（or）anxiety symptoms in all NERD patients was 82.5%.The major clinical symptoms,depressive symptoms and（or）anxiety symptoms improved significantly in the treatment groups（P0.01）after 8 weeks.Conclusion There is a high rate of depressive and anxiety symptoms in NERD patients.Psychological counseling are effective in relieving both somatic and psychological symptoms in NERD patients.
