区域性热疗合并热化疗治疗腹腔转移瘤的临床研究

摘　　要：目的 :介绍一种区域性加温联合多途径给药化疗治疗腹腔转移瘤的方法及其毒副作用。方法 :用UHR 915MHz微波热疗机对 11例腹腔转移瘤病人进行 12次腹部加温 ,并对直肠、食管、和鼓膜进行测温 ,加温前给予度冷丁、氯丙嗪和东莨菪碱肌注 ,给予腹腔灌注 4 8℃～ 50℃的DDP溶液 12 50ml ,并根据肿瘤部位 ,部分患者给予介入化疗 ,加温的同时和加温后给予足量静脉化疗。结果 :经测温显示各部位温度于加温后达 4 0℃以上所需中位时间分别为 81(2 4～ 84 )min(鼓膜 7次 )、81(2 4～ 93)min(食管 7次 )、4 2 (9～ 72 )min(直肠 10次 ) ;达 4 1℃中位时间是 72 (39～ 96 )min(鼓膜 4次 )、72 (4 2～ 10 2 )min(食管5次 )、72 (9～ 10 2 )min(直肠 7次 ) ;4 0℃以上持续时间分别是鼓膜 57(36～ 96 )min ,食管 39(2 7～ 96 )min ,直肠 6 6 (4 8～ 111)min ;各部位最高温度分别是 4 0 .4 (39.4～ 4 1.3)℃、4 0 .4 (39.5～ 4 2 )℃、4 1.5(39.2～4 3)℃。毒副反应 :少量～中等量出汗 ,心率增快 ,心电图改变为S T段低平或轻度下移 ,可以自行恢复。4例病人加温局部充血 ,2例出现水泡 ,1例皮下脂肪硬结 ,3例治疗结束后 6h出现恶心、呕吐 ,11例白细胞下降 1～ 2级 ,1例出现消化道大出血 ,上述毒副反应经对症治疗后缓解。Objective:To introduce a method of regional microwave hyperthermia combined with thermo chemotherapy in treating the metastatic tumors of abdominal cavity,and explore its adverse effects.Methods:11 cases with abdominal metastatic tumor were studied.First,1250ml CDDP solution with 48℃-50℃ was injected to abdominal cavity,while other chemotherapeutic agents were given intravenously.Then,the microwave with 915 MHz was put on umbilicus or other diseased part 120min to keep the temperature above 40℃.Each was 12 times.The temperature in rectum,esophagus and tympanum was detected in turn.Results:Of 11 cases,required median(Md)time of reaching 40℃ in tympanum,esophagus and rectum was 81 min,81 min and 42 min respectively after heating;Above 41℃,it was 72 min,72 min and 72 min.The Md of the highest temperature in each part was in turn 40.4℃,40.4℃ and 41.5℃.Occasionally,some adverse effects were presented,such as sweating,and leucopenia (which was found in all cases).Conclusions:It suggest that microwave regional hyperthermia combine with thermo chemotherapy way rapidly raise the body temperature to killing the tumor cell with limited side effects.
