摘 要:目的:通过回顾性研究,探讨不同腭裂手术年龄对上颌骨生长发育的影响。方法:将101例年龄在10岁以上,腭裂术后2年以上的单侧完全性唇腭裂患者,根据接受腭裂手术的年龄分为3组。1组:0~3.00岁手术组(n=37)、2组:3.01~6.00岁手术组(n=35),3组:6.01岁以上手术组(n=29)。随访时行头影测量,对其上颌骨生长发育进行评价,并对不同手术年龄组间上颌骨生长发育各测量指标间的差异进行统计学分析。结果:SNA、SNB、ANB、ANS-Ptm、N-ANS'/N-Me等测量指标的校正值在三组间均无显著性差异(P>0.05),只有Ba-A的校正值在A组和C组间有显著性差异(P<0.05);但是SNA、ANB、Ba-A三指标的均值随手术年龄的增大而增大。结论:上颌骨前后向生长发育随手术年龄的增大而受抑制的程度有减小的趋势,腭裂手术不但会阻碍上颌骨位置的前移,而且会抑制上颌骨长度的生长,但对上颌骨高度生长的抑制作用有限。Objective To investigate the effect of different palatoplasty timing on maxillary development by a retrospective study. Methods 101 UCLP consecutive cases who were older than 10 years were followed up,which were divided into three groups according to the age of operation, first group:0-3.00 years old, second group:3.01-6.00 years old and third group:6.01 years old above. Lateral cephalogram were taken for each patient,and cephalometric analysis were used for assessing maxillary growth. Results The results of cephalometric show that there was no significant difference among three groups for all variable (∠SNA, ∠SNB, ∠ANB,ANS-Ptm,N-ANS'/N-Me) except Ba-A variable, significant difference existed between 1st and 3rd groups for Ba-A variable justly. However, the mean value of /SNA, Z.ANB and Ba-A increased along with increase of operation age. Conclusion The maxillary development was better along with increase of operation age.Not only the maxillary position,but also the maxillary length will be hindered by the cleft palate surgery.
