内窥镜辅助在颌骨骨折经穿颊器微创坚固内固定中的初步应用

摘　　要：目的:尝试将内窥镜辅助应用于颌骨骨折经穿颊器微创坚固内固定术中。方法:通过11例下颌角、下颌骨乙状切迹至升支后缘骨折患者,4例颧弓骨折患者,尝试应用口内切开内窥镜辅助显露骨折线和自制穿颊器口外穿刺内固定治疗下颌骨升支及颧骨骨折,手术前后常规照面像和咬合像、X线片。术后2周拍左侧面像、咬合像,拍X线片观察。结论:面部无肿胀、无面瘫及涎漏发生,术后2周复诊面部无瘢痕。咬合关系完全恢复,X线片上断端对位、对线良好。结果:借助内窥镜可以在以往传统口内入路时不易达到的下颌骨角部下缘、下颌升支后缘处以及乙状切迹、髁突颈部骨折及颧骨颧弓新鲜骨折进行内固定,术后固定效果可靠,手术创伤明显减少,取得了良好的治疗效果。Objective To evaluate effectiveness of internal-fixation in maxillofacial fracture with endoscope and transbuccal trochar. Methods Fifteen patients including mandibular angle,ramus and zygoma fractures were fixed with transbuccal trochar and endoscope.All patients were performed the imaging examination preoperatively and postoperatively. Results All patients showed no swelling,facial paralysis and salivary fistula.2 months after operation, there is no visible facial scars and they recoverd normal occlusion. Conclusion The interal-fixation under endoscope and transbuccal trochar is an effective way of treating troublesome fracture of mandibular angle, ramus, condyle and zygoma, with little surgery trauma.
