摘 要:目的:研究2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者血浆丝氨酸蛋白酶抑制剂(vaspin)水平与代谢指标的关系。方法:采用酶联免疫吸附法(ELISA)检测T2DM合并NAFLD组(A组)、T2DM不合并NAFLD组(B组)和正常对照(NC)组的血浆vaspin水平,采用间接测热法检测能量代谢指标,同时测定相关的临床指标。结果:B组血浆vaspin水平高于A组和NC组,A组和B组呼吸高(RQ)值和碳水化合物氧化率显著低于NC组,而24h尿素氮、静息代谢率(REE)、REE/体质量和脂肪氧化率显著高于NC组(P<0.05或P<0.01),A、B组间差异均无统计学意义(P>0.05)。A组血浆vaspin水平与体质量、体质量指数(BMI)、REE和性别(男=23,女=20)呈正相关,与RQ呈负相关;B组血浆vaspin水平与BMI、腰臀比(WHR)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、REE、REE/体质量、TG呈正相关,与RQ呈负相关。vaspin值为92.4μg/L时可作为预测2型糖尿病患者脂肪肝的风险值。结论:血浆vaspin水平与物质及能量代谢密切相关,并可用来预测2型糖尿病患者脂肪肝的风险。Objective: To investigate the relationship between the serum vaspin and the metabolic index in nonalcoholic fatty liver disease (NAFLD) patients with type2 diabetes mellitus (T2DM). Methods: The level of the serum vaspin was assayed by enzyme-linkod immunosorbent assay (ELISA) in patients of T2DM with NAFLD (group A), T2DM without NAFLD (group B)and normal control group (group NC). The energy metabolism indexes were detected by indirect calorimetry. The clinical data were also detected. Results: The level of serum vaspin was significantly increased in group B than that of group A and group NC (P 〈 0.01). The respiratory quotient (RQ) and oxidation of carbohydrate were significantly lower in group A and group B than that of group NC. Levels of 24 hr urine nitrogen, real resting energy expenditure (REE), REE/body weight and the oxidation of fat were significantly higher in group A and group B than those of group NC (P 〈 0.05 or P 〈 0.01), but there was no significant difference between group A and group B (P 〉 0.05). The level of serum vaspin was positively correlated with body weight, body mass index (BMI), REE and gender; meanwhile, it was negatively correlated with RQ in group A. The level of serum vaspin was positively correlated with BMI, waist-to-hip ratio (WHR), fasting blood glucose (FBG), fasting insulin (FINS); homeostasis model assessment of insulin resistance (HOMA-IR), real REE, real REE/body weight and triacylglycero] (TG), while it was negatively correlated with RQ in group B. The concentration of vaspin, 92.4 μg/L could predict the risk of T2DM with fatty liver. Conclusion: The level of serum vaspin was closely related with the substance and energy metabolism, which could be used to predict the risk of type 2 diabetes mellitus with fatty liver.
