摘 要:目的:观察参麦注射液联合全身化疗(GP)治疗晚期非小细胞肺癌(NSCLC)的近期疗效和不良反应,探讨治疗前后患者血清TNF-α水平的变化及临床意义。方法:50例晚期NSCLC患者随机分为两组。治疗组25例,参麦注射液50mL加入5%葡萄糖注射液250mL中静脉输注d1~10,联合GP方案全身化疗,吉西他滨1000mg/㎡静脉滴注30min,d1、8,顺铂25mg/㎡静脉滴注2h,d1~3,每3周重复;对照组25例,仅行GP方案全身化疗;两个疗程为1个评估周期,观察近期疗效和不良反应。同时采用双抗夹心酶联免疫吸附法(Elisa)检测50例患者治疗前后血清TNF-α的含量。结果:治疗组和对照组有效率(CR+PR)分别为44%vs 36%,无显著差异(χ2=0.33,P=0.56);疾病控制率(CR+PR+SD)分别是84%vs 60%,有显著差异(χ2=3.87,P=0.049),不良反应治疗组较对照组明显减轻(P<0.05)。治疗组和对照组治疗后血清TNF-α水平较治疗前均有显著下降(P<0.05),下降幅度治疗组较对照组更加明显有统计学意义(P=0.016);而治疗无效者治疗组前后血清TNF-α差异有显著性(P=0.013),对照组无显著性差异(P=0.097)。结论:参麦注射液联合全身化疗能够显著降低晚期NSCLC患者血清TNF-α水平,提高疾病控制率,减轻化疗不良反应,改善生活质量。Objective :To evaluate the response rate in patients with advanced non- small cell lung cancer( NSCLC ), who were treated with Shenmai injection plus gemcitabine and cisplatin( GP combined group )or single GP( chemothera- peutic group) and to compare the levels of TNF - α in serum of the patients before and after treatment. Methods : A total of 50 patients with stages Ⅲ and Ⅳ NSCLC were randomly assigned into two trial groups:25 patients in combined group (Shenmai injection 50mL added into 250mL of 5% glucose injection for intravenous infusion on dl - 10, gemcitabine 1000mg/m: for intervenous drop infusion for 30min on day 1 and 8 ,cisplatin 25mg/m2 for intravenous infusion for 2h on day 1 to 3, every 3 weeks these therapies were reused. The control group was only given GP regimen alone. 2 cycles was an evaluating period. The concentrations of the NF - α level in serum were measured by enzyme - linked immunosorbent assay respectively in patients with NSCLC before and after treatment. Results:The response rate in combined group( CR + PR)was 44% ;in the chemotherapeutic group was 36%. There was no significant difference between two groups(χ2 = 0. 33 ,P =0.56). The disease control rate in combined group (CR + PR + SD) was 84 % ;in the chemotherapeutic group was 60%. Disease control rate of the two groups were significantly different(χ2 = 3.87 ,P = 0.049). Tiae adverse reaction of the combined group was significantly fewer than that of the chemotherapeutic group( P 〈 0.05 ). The TNF - α levels in serum of two groups were significantly decreased after treatment ( P 〈0. 05 ), and the range of decline in the combined group was more apparent(P = 0. 016). In ineffective patients, the TNF - α levels in serum was also significant- ly decreased in combined group ( P = 0. 013 ) , but not in the combined group ( P = 0. 097 ).
